Attachment #10 Representations and Certifications Attachment #9 Form SF1449 Attachment #8 Proposal Packaging and Delivery Instructions Attachment #7 Invoicing Instructions Attachment #6 Technical evaluation Criteria Attachment #5 Past Performance Questionnaire Attachment #4 Proposal Preperation Instructions Attachment #3 Information Sercurity Requirements Attachment #2 Description of Supplies/Services & Price/Costs Attachment #1 Statement of Work The Clinical Center at National Institutes of Health proposes to procure System Administration Services to help install the equipment necessary to create a Clinical Data Mart, in accordance with the procedures for acquiring commercial items/services authorized in FAR Part 12. The Data Mart will create a system redundancy that will enable the Critical Clinical Systems to be bought back in line within 24 hours or less. This is a Combined Synopsis for Commercial Services prepared in accordance with FAR Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; a separate solicitation will not be issued. This solicitation NIHCL2010047 includes all provisions and clauses in effect through Federal Acquisition Circular 2005-38 date December 10 2009. The acquisition will be made pursuant to the authority in FAR 13.5 to use simplified procedures for commercial requirements. The acquisition is set aside for small businesses. The standard North American Industry Classification System (NAICS) code is 541511 and the size standard is 25 million dollars. This solicitation is being issued as a Request for Proposal and is a total small business set-aside. NIH is soliciting offers for the continuing development, implementation and maintenance of the Patient Recruitment and Public Liaison (PRPL), Data Mart Hospital Services and Data Transformation Initiative (DTI) applications to support the Department of Clinical Research Informatics (DCRI) operation and user support mission. These requirements include but are not limited to requirements, documentation, application and database development, system implementation and maintenance. In order to provide data to the Institutes from the CRIS there is a need to create a Clinical Data Mart. The CRIS Data Mart will contain the data from the current MIS Data Mart as well as the new data from the CRIS electronic health records. A backup of the CRIS transactional data base is done each night and the plan is to restore a copy of this backup for use with the CRIS Data Mart. The PRPL Office has two PowerBuilder applications that are currently being updated and converted to a web application. The Volunteer Payment portion of the application has been converted and is in production. The Protocol, Recruitment and Contact Management Modules still need to be completed. The Data Transformation Initiative is a Clinical Center project to improve the automation and access to departmental data in order to improve the accuracy of protocol resource utilization and implement industry standard coding to support revenue generation. The Hospital Services application was developed six years ago and provides NIH staff a means of ordering hospital supplies, Positron Emission Tomography (PET) products and pharmacy floor stock. This production application requires additional modules and occasional modifications and maintenance The detailed Statement of Work requirements can be found in Attachment No. 1 to this synopsis. The requirements stated in the Statement of Work list very specific requirements that must be addressed in your proposal and these requirements will be a critical part of the evaluation process. The period of performance is for a period of six months commencing at date of award, with two option periods; one from the conclusion of the base period for a duration of six months, the second commencing at the end of option period one for a duration of six months. Please refer to the following attachments in preparing your proposal responding to this solicitation.
