Vendor Survey Form Photo No. 4824591452 LC Drawing Specification #040-1452900 Solicitation No. LCNLS10Q0018 SYNOPSIS: The Library of Congress, National Library Service for the Blind and Physically Handicapped (NLS) has a requirement for the production of Styrofoam Inserts, Digital End Cap (SH137) in accordance to LC drawing specification #040-1452900, and ship order to two designated locations within the USA upon receipt of approval from NLS/BPH of the producer’s provided production samples. QUESTIONS: Offerors must submit all questions concerning this solicitation in writing to the Contract Specialist via email at lsne@loc.gov. All such questions must be received no later than February 9, 2010; 11:00AM (EASTERN). All responses to questions will be made in writing and included in an amendment to the solicitation. Proposals shall be submitted via email to lsne@loc.gov or fax to (202) 707-8611. ALL PROPOSALS SHALL BE SUBMITTED BY FEBRUARY 16, 2010; 10:00AM (EASTERN). The Library reserves the right to award without discussions.
