- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The type contract anticipated is firm-fixed price. Solicitation Number FM2855 9253 0019 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38 (effective 1 February 2010) and Defense Federal Acquisition Regulation Supplement change notice 20091229. This is a sole source requirement in accordance with FAR 6.302-1. Intended source: LKC Technologies Inc. Reason: Maintain consistency and compatibility with present testing methodology and translation from old system to new system; allows transfer of data retrieved from our present system to this system. All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the agency. The NACIS Code is 339115; the size standard is 500 people. Line Item 0001 - Physiologic Analyzer - 1 each Description: UTAS Sunburst Visual Diagnostic Testing System. Simulators: SunBurst High Brightness Color LED Full Field (Ganzfeld) Stimulator with built in IR Fixation Camera and fully adjustable mounting arm. Computer: Fully configured PC with Windows XP Pro. Includes mouse, LED Illuminated Keyboard, LCD Operator Monitor, Color Inkjet Printer, CD/DVD RW. Data Acquisition: UBA-4204 Ultra Low Noise, Fully Isolated Digital Biomedical Amplifier (4 recording channels) with USB Interface. Medical Grade Isolation Transformer. Software: EMWinF, EM for Windows Software. EMWinzUT, Additional Protocols for Double Flash ERG. Self-Calibration Software for SunBurst Ganzfeld. Electrodes: 20 ERG Jets, 3 VEP/VER (10mm cup, gold plated), 2 Ear Clips (10mm cup with clip), 1 Ground Reference Lead, 4 EOGs (8mm Ag-AgCI), Disposable Ground Electrodes (box of 30). Supplies: 1 tube Electrode gel (8 oz), 1 tube Electrode Cream (3.5 oz), Skin Cleanser Pads (box of 100), 100 EOG Electrode Washers, 1 Red Light Bulb, 1 LKC Led Keychain. Technical capability will be evaluated by how well the proposed product meets the government requirement instead of predetermined sub-factors. Evaluation Basis for Award: Award will be made to the offeror who has the lowest priced, technically acceptable proposal.
