- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and is unrestricted. Proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442792990805 and is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38 effective 1 Feb 10; Defense DCN 20100120 (Amended) Edition, and AFAC 2009-1030 effective 30 Oct 09. The North American Industry Classification System (NAICS) code is 423450. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 5047. Travis AFB has a requirement for a Class IV LCT-1000 Laser System. Description: The laser requires no FDA Warning letters and the laser must be ISO Certified. This ISO certification ensures standardization of quality, safety, reliability, and efficiency. This laser must have 10 watts of continuous power and be unfilitered and unconverted. The laser must be able to penetrate tissue to a depth of 10 cm or greater. The laser must have a dual wavelength capability: one in the low 800 nm and one in the high 900 nm range. A massage capability is required to help release adhesions, reduce pain, and reduce spasm in conjunction with the laser to make the treatment more effective. A foot switch will help increase safety with on/off power control. Must provide one-time inservice training for 27 people at the Physical Therapy clinic at Travis AFB. Technical support to be for the specified period of warranty. Qty: 1 CLIN: 0001-AA Laser, diode, biosimulation. Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition.
