Description
REVISED Appliance Maintenance combined synopsis/solicitation to reflect changes in quantities identified in revised SOW. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation, FA2517-10-T-5004, is being issued as a Request for Quotation (RFQ). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38. (iv) This procurement is a small business set aside. The NAICS code is 238990, All Other Specialty Trade Contractors, and the size standard is $14 million. (v) Contractor shall utilize the below line item structure when preparing quotes: Line Item 0001 Quantity Unit Unit Price Amount 2 EA ________ ________ Description: Appliance Preventative Maintenance-Dorms Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform preventive maintenance on 71 washers, 82 dryers, 208 refrigerators, 205 cooktops and 2 full ranges and 4 ice machines in 4 dorms In Accordance With (IAW) attached Performance Work Statement (PWS). Preventive maintenance is to be accomplished semi-annually. Line Item 0002 Quantity Unit Unit Price 1 HR __________ (DURING DUTY HOURS) 1 HR __________ (AFTER DUTY HOURS) Description: Appliance Service, Repair, and Parts-Dorms Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform service and repair, and to provide parts on 71 washers, 82 dryers, 208 refrigerators, 205 cooktops and 2 full ranges and 4 ice machines in 4 dorms IAW attached PWS. Provide hourly rate for service calls during duty hours (7:30 am - 4:30 pm Mountain Standard Time (MST)) and an hourly rate for service calls after duty hours. Line Item 0003 Quantity Unit Unit Price Amount 2 EA ________ _______ Description: Appliance Preventative Maintenance-Lodging Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform preventive maintenance on 55 Stackable Washer/Dryers, 2 sets of side-by-side Washer/Dryers, 107 Stove/Ovens, 68 Dishwashers, 4 Ice machines, and 143 Refrigerators IAW attached PWS. Preventative maintenance is to be accomplished semi-annually. Line Item 0004 Quantity Unit Unit Price 1 HR __________ (DURING DUTY HOURS) 1 HR __________ (AFTER DUTY HOURS) Description: Appliance Service, Repair, and Parts-Lodging Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform service and repair, and to provide parts on 55 Stackable Washer/Dryers, 2 sets of side-by-side Washer Dryers, 107 Stove/Ovens, 68 Dishwashers, 4 Ice machines, and 143 Refrigerators IAW attached PWS. (vi) The following commercial item is requested in this solicitation: Non personal services to provide all labor, parts, tools, materials, equipment, personnel, supervision, transportation, and other items or services necessary to perform maintenance and repair on appliances in the Dorms and Lodging at Peterson AFB, CO IAW the attached PWS. Period of performance is from 15 February 2010 - 14 February 2011. (vii) Advanced payments are not authorized. To be performed on Peterson AFB, CO from 15 February 2010 - 14 February 2011. (viii) The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors/Commercial Items. All firms must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. All firms must be registered in the Central Contractor Registration Database at www.ccr.gov in order to be considered for award. (ix) FAR 52.212-2(a) Evaluation - Commercial Items, applies to this acquisition. Quote will be evaluated based on price. (x) The provision at FAR 52.212-3, Offeror Representations and Certification-Commercial Items, is applicable to this acquisition. All vendors must be registered in Online Representations and certifications (ORCA) (https://orca.bpn.gov) at the time of Quote submittal. (xi) The clause at FAR 52.212-4 (Alt. 1), Contract Terms and Conditions-Commercial Items, applies to the acquisition. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items (Dec 2009) (Deviation), applies to this acquisition. (xiii) The clauses FAR 52.237-2, Protection of Government Buildings, Equipment, and Vegetation; DFARS 252.204-7004, Required Central Contractor Registration. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders; DFARS 252.232-7003, Electronic Submission of Payment Requests; AFFARS 5352.201-9101, Ombudsman (as prescribed 5301.9103); AFFARS 5352.223-9001, Health and Safety on Government Installations; AFFARS 5352.242-9000, Contractor Access to Air Force Installations (xiv) Quote Submission: include price information. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. Contractor must be registered in CCR, ORCA and WAWF to be considered. The quote should provide for a cost of each line as described in paragraph (v). (xv) Site visit will be conducted at 0900 on 1 February 2010.
