- The U S Naval Hospital, Guam intends to enter into negotiations with LMD Corporation, dba: Promotions Specialties, 278 South Marine Drive, Suite 201, Tamuning, Guam 96913 with the intent of awarding a sole source in accordance with FAR 6.302-1 and FAR 13-106-1(b). The resulting contract will provide all necesary labor, materials, transportaion, supervision, equipments, engineering designs and other incidential work required for this project. A determination by the Governemnt not to compete with this requirement based upon the previous project to maintain the consistancy and warranties. The notice of intent is not a request proposals. (see statement of work below) Facilities Management Department Interior Signage for U.S. Hospital, Guam Bldg #1; U.S. Naval Branch Dental Clinic, Bldg #1; and U.S. Naval Branch Medical Clinic, Bldg #6 Statement of Work 1.0PROJECT DESCRIPTION: This project is to update interior signage at Building 1, U.S. Naval Hospital, Guam, Building 1, U.S. Naval Branch Dental Clinic, and Building 6, U.S. Naval Branch Medical Clinic, COMNAVMAR, Guam. 2.0LOCATION OF WORK: The work is located at Building 1, U.S. Naval Hospital, Guam, Building 1, U.S. Naval Branch Dental Clinic, and Building 6, U.S. Naval Branch Medical Clinic, COMNAVMAR, Guam. See attached location and site plans. 3.0SCOPE OF WORK: Provide all necessary labor, material, transportation, supervision, equipment, engineering design, and other incidental work required for this project. Other incidental work required includes any and all items and considerations necessary to insure a complete and usable final product, including, but not limited to, the necessary design and construction considerations not specifically stated elsewhere in this statement of work. "Complete and usable final product" means that the completed final product can be used to fully satisfy the requirements and the intended purpose of the project. The scope of work includes, but is not limited to, the following: 1)Building 1, U.S. Naval Hospital a)Architectural Work - Interior [1aa]Remove and dispose all existing room signage, except Wing D2, all existing stairway signage, and all existing elevator door number signage. [1ab]Remove and dispose all existing Unit Signage [1ac]Remove and dispose all existing Department signage [1ad]Remove all existing Operation Hour Signage [1ae]Remove and dispose all “NO SMOKING” signs [1af]Clean and paint wall area behind removed signs to match existing conditions. [1ag]Provide double sided flag mount signs on all requested areas including restrooms, barber shop, cafeteria, NEX, and command officer, etc., to be visible from corridor. [1ah]Provide Room and User Signage with Insert Strips, see Drawing Sheet No. 10, at the entrance of each room and stairway, identified in Table 4. [1ai]Provide Unit Signage and Unit Directory, see Drawing Sheet Nos. 7 and 8, at the entrance of each wing identified in Table 2. [1aj]Provide Department Signage, see Drawing Sheet No. 6, at the entrance of each department identified in Table 1. [1ak]Provide Operation Hour Signage at each main entrance to the hospital. [1al]Provide Door Number signage, see Drawing Sheet No. 11, for each elevator door identified in Table 3. [1am]Provide “NO SMOKING” signage at all existing “NO SMOKING” sign locations and entrance doors. Note: Due to operational conditions, the following locations require after hour (17:00 ~ 20:00) and/or weekend (Saturday or Sunday) work: Wing E2 and E3, Wing F2 and F3, and Wing J3. 2)Building 1, U.S. Naval Branch Dental Clinic a)Architectural Work - Interior [2aa]Remove and dispose all existing room signage [2ab]Remove and dispose all “NO SMOKING” signs. [2ac]Clean and paint wall area behind removed signs to match existing conditions. [2ad]Provide double sided flag mount signs on all requested areas including restrooms, Exam and X-Ray Room, Exam Rooms, etc., to be visible from corridor. [2ae]Provide Room and User Signage with Insert Strips, see Drawing Sheet No. 10, at the entrance of each room, identified in Table 5. [2af]Provide Floor Directory at the main entrance to Bldg. 1. [2ag]Provide “NO SMOKING” signage at all existing “NO SMOKING” sign locations and entrance doors. 3)Building 6, U.S. Naval Branch Medical Clinic a)Architectural Work - Interior [3aa]Remove and dispose all existing room signage and stairway signage [3ab]Clean and paint wall area behind removed signs to match existing conditions. [3ac]Provide double sided flag mount signs on all requested areas including restrooms, Laboratory, Optometry, Record’s Room, Exam Rooms, X-ray Office, Chief Office, Preventive Medicine Office, Pharmacy Room, internal stairway, etc., to be visible from corridor. [3ad]Provide Room and User Signage with Insert Strips, see Drawing Sheet No. 10, at the entrance of each room and stairway identified in Table 6. [3ae]Provide Floor Directory at the main entrance of each floor to Bldg. 6. Note: All signage shall be uniform in size, type of letter, and color similar to updated conditions at Wing D2 of Building 1, U.S. Naval Hospital, and be in compliance with the American with Disabilities Act standards. All existing building conditions, e.g. rooms, units, departments, floors, room/door numbers, and room signage, must be field-verified by the Contractor. Coordinate with Facility Management Department concerning work hours and design requirements including consistency in numbering, installation, and signage type. 3.1CONSTRUCTION REQUIREMENTS: In addition to the requirements of the basic contract, comply with Marianas Region Architectural and Construction Standards (MRACS). 3.2ENGINEERING SYSTEMS REQUIREMENTS: See part 4 for minimum requirements. 4.0SUBMITTALS: In addition to the submittals required under the basic contract, submit the following: Proposal requirements: •Narrative of the proposal, indicating, in detail, the design and materials considered significant for this project for technical evaluation purposes, •Detailed cost estimate, •Preliminary construction schedule. 5.0SITE SAFETY AND HEALTH OFFICER (SSHO) QUALIFICATIONS: The Site Safety and Health Officer for this Task Order shall be on the site during construction period. 6.0QUALITY CONTROL (QC) MANAGER QUALIFICATIONS: The QC Manager for this Task Order shall be on the site during construction period. 7.0PERFORMANCE PERIOD: 120 calendar days.
