CONTRACT SOLICITATION: VA-260-10-RP-0111 ISSUE DATE: February 12, 2010 OWNER: Federal Government - Veterans Health Administration VISN 20 Medical Services Contracting Vancouver, OR CLIENT (Parent Facility): Walla Walla VA Medical Center Jonathan M. Wainwright Memorial VA Medical Center 77 Wainwright Drive Walla Walla, WA 99362 PURPOSE: Primary Care Outpatient Medical Services and Associated Facility Space for eligible Veterans LOCATION OF CONTRACTED SERVICES: Kamiah (Lewis County), ID PREFERENCE: Existing, fully operational, community based clinic or hospital to absorb our eligible vets simultaneously with the general public who are currently being examined at same facility TARGET AWARD DATE: June, 2010 1.TYPE of CONTRACT: Firm Fixed Price, Requirements 2.PRICING: All-Inclusive, Capitated Rate per number of estimated, eligible Vets x 12 months = Annual and Grand Totals, as approved thru Annual Congressional Appropriations. 3.TERM: Base Year (First 12 months after Award date), with possibility of Four, 12 Month Annual Renewal Options ADMINISTRATOR/CONTACT: Earl Dierking, Contract Specialist 360-852-9865 Earl.Dierking@VA.GOV.
