- Product Code; 80-1170 (to replace 80-1187), CMCIII Malis Bipolar Generator; This solicitation is restricted (set-aside) to small businesses. Requirement to be brand name or equal basis. Items on this RFQ are: Product Code; 80-1170 (to replace 80-1187), CMCIII Malis Bipolar Generator; Product Code: 80-9100, Module 1000 Irrigator; Product Code: 80-9114, Y-Connecting Product Code: 80-9102, Module 1000 Integrated Tubing/Cord Set For the Main Operating Department manufactured by Johnson & Johnson - Codman. Quotes should reflect pricing on all items for consideration. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). All items are to be warranted against manufacturer's defects. Vendor must replace all defective items at no additional cost to the Government. Vendor's price to be inclusive of all cost including shipping. Contractor shall supply a current published price list or GSA/VA Federal Supply Schedule contract for supplies &/or services being procured. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on Friday, 26 March 2010.
