Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation, but market research to determine potential sources for the lease/rental of a Premier Shockwave Ossatron Excorporeal Shockwave Therapy - HIGH ENERGY System (Brand Name or Equal). The Ossatron System must be able to emit a series of high-energy acoustic impulses (shock waves) to provide a non invasive surgical alternative for patients diagnosed with chronic plantar fasciitis. The Ossatron system shall be able to perform the following procedures: Unilateral Tendinopathy, Bilateral Tendinopathy, and Non-union. The contractor shall provide all equipment (excluding consumables, accessories, and operating supplies) for the equipment to function properly while conducting the above procedures. The vender shall furnish and replace all repair parts if the equipment does not function properly, or malfunctions during use, at no additional cost to the government. Services shall include: All unscheduled/scheduled maintenance will be provided by the vender. The vender will be responsible for delivery/pickup of the Premier Shockwave Ossatron Excorporeal Shockwave Therapy - HIGH ENERGY System upon request from the customer located at 73 Nealy Ave, Bldg 256, Langley AFB, VA 23665, Attention Medical Maintenance, between the hours of 7:30 to 4:30 p.m. excluding federal holidays. The vender will provide 633d Medical Maintenance a valid calibration certificate that meets the manufactures recommended calibration intervals upon delivery of the System. The period of performance will be for 1 base year plus four options. An estimated twelve to fifteen procedures will be performed every quarter. The NAICS code for this procurement is 532490. The SIC code is 7352. This sources sought synopsis neither constitutes a Request for Proposal (RFP), nor does it restrict the Government to an ultimate acquisition approach. This synopsis should not be construed as a commitment by the Government for any purpose. Requests for a solicitation will not receive a response. Interested sources should respond to this synopsis by submitting (1) Name, Address, and phone number of the firm (including CAGE code); (2) Size status; (3) provide literature (qualification package) that describes their product technical capabilities; their understanding of the Premier Shockwave Ossatron Excorporeal Shockwave Therapy - HIGH ENERGY System requirements and their. The Government will assess prospective sources' potential based on the qualification packages provided. Firms considered by the Government to be most highly qualified to deliver the Premier Shockwave Ossatron Excorporeal Shockwave Therapy - HIGH ENERGY System (solution will be contacted by the PCO, and a list of such firms will be made available on the Federal Business Opportunities website. Firms considered by the Government not to be most highly qualified to deliver the Premier Shockwave Ossatron Excorporeal Shockwave Therapy - HIGH ENERGY System will be afforded an opportunity to obtain the Government's rationale and offered the opportunity to provide additional information. Any interested source initially evaluated as not most highly qualified will be provided a copy of the solicitation upon request, should one be issued, and any offer submitted by such a firm would be evaluated without prejudice. Firms should indicate whether they are a large business, small business, small disadvantaged business, 8(a) concern, and woman-owned small business, whether they qualify as socially or economically disadvantaged, and whether they are a U.S. or foreign-owned firm.
