- SECTION E - SOLICITATION PROVISIONS ADDENDUM TO 52.212-1 INSRTRUCTION TO OFFERORS-COMMERCIAL ITEMS. (APR 2008) PROPOSAL CONTENTS 1. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. This section specifies the format and content that offerors shall use in this Request for Proposal (RFP). The intent is not to restrict the offerors in the manner in which they will perform their work but rather to ensure a certain degree of uniformity in the format of the responses for evaluation purposes. Offerors must submit a proposal that is legible and comprehensive enough to provide the basis for a sound evaluation by the Government. Information provided should be precise, factual, current and complete. Legibility, clarity, completeness, and responsiveness are of the utmost importance. Proposals shall be in the form prescribed by, and shall contain a response to, each of the areas identified below. Any proposal which does not provide, as a minimum, that which is required in this solicitation may be determined to be substantially incomplete/non-compliant and not warrant any further consideration. Proposals must be completed and self-sufficient. The response shall consist of three (3) separate submittals as follow: PART I - Pricing Proposals (this solicitation document duly completed); and PART II- Staffing and Experience (Personnel Qualification Sheet (Attachment A) and Curriculum Vitae/Resume) PART III- Past and Present Performance Questionnaire (Attachment B) PART I - Pricing Quote: Completed Standard Form 1449 with particular attention to blocks 17a, 17b, and 30a through 30c of page 1. The offeror shall submit a price for each individual Representative Contract Line Item Number (CLIN) beginning on page 2. A signed Authorization to Release Information, VA Form 10-0149, Attachment "C". Completed Representations and Certifications (52.212-3 Alt I). Acknowledgement of any amendments issued by the Government. Each amendment shall be acknowledged by completing blocks 15A, 15B, and 15C of the SF 30 (amendment). PART II- Experience and Staffing Provide a list of names of the qualified personnel intended to be utilized in the performance of this contract and/or describe the mechanism used to provide qualified personnel to perform the contract work. Provide documentation that demonstrates that contractor's personnel performing sonographers services have no less that four years minimum full time experience in each discipline performed. Provide a fully filled out Personnel Qualification Sheet (Attachment A 4 of 4 pages) which includes question regarding personnel past and relevant experience, education, and licensure and must be completed as part of your submission for contractor Sonographers and planned substitute Sonographers must also be made part of your submission. Along with the Personnel Qualifications Sheet provide a copy of each candidates resume, and copies of certifications needed for anyone to be assigned to this contract. Evidence of registered status in each respective area (RDMS, RVT, RDCS) and compliance with current standards of continuing education must be provided. Include documentation of experience with the accreditation process for non-invasive vascular and general diagnostic ultrasound and provide all required administrative functions for acquisition of accreditation. Previous VA experience is preferred; however candidates with no previous VA experience shall not be disqualified. List the number of administrative support staff and describe the level of training and experience that shall be utilized to meet the administrative support functions of this contract. Describe your organization structure and management practices relative to the requirements contained in the contract. IMPORTANT NOTE: Substitution of personnel prior to or after contract award may occur only with the advanced notification and approval of the Contracting Officer. 2. CORRESPONDENCE (questions, submittals, etc.). The Department of Veterans Affairs requires at least three past and present performance references for the most recent contracts you have performed. Attachment A, Past and Present Performance Questionnaire, must be completed and made part of your submission:.
