- Q-SOURCES SOUGHT NOTICE ONLY: Breast Health ServicesThis is a request for information only and no solicitation currently exists; therefore, DO NOT REQUEST A COPY OF THE SOLICITATION as there is no assurance that a solicitation will result. The government will not pay for any costs associated with responses submitted for this Notice. The Department of Veterans Affairs, Veterans Integrated Service Network (VISN) 1, VAMC Manchester New Hampshire has a requirement for Breast Health Services for Veteran outpatients in the New Hampshire area. These services are required for approximately 350 Veteran patients receiving care at the medical center and the Community Based Outpatient Clinics (CBOC's). The intent of the requirement is to award Multiple Indefinite-Delivery Indefinite-Quantity (IDIQ) contract awards. This notice is published to conduct market research and to determine if there are sufficient number of potential sources capable of providing the required services and to assist the Government in determining whether or not this procurement warrants a set aside for small business, a small business program i.e. Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB), or accomplished via full and open competition in accordance with the Federal Acquisition Regulation (FAR), Clause 19.502-2 (b). The term of the IDIQ Multi-Award Contract may be one year base period from date of award, with four (4) one (1) year options to extend or may be (1) two year base period from date of award, with three (3) one (1) year options to extend. The applicable NAICS code is NAICS Code 621111- Office of Physicians; contractors have a size standard of no greater than 50 percent of NAICS Code with a Small Business Size Standard of $10.0M. Please note that this NAICS Code 621111 - Offices of Physicians does not encompass medical personnel staffing agencies or temporary help services. The type of work the Contractor will be required to perform is breast health procedures/services. These procedures/services include but are not limited to the following: 1. Mammograms Screening and Diagnostic with Interpretations 2.Breast Ultrasounds 3.Breast MRI with Interpretations 4.Breast Interventional Services The contractor is to furnish all labor, material and equipment necessary to provide the various breast examinations and interpretation services for, mammogram screenings/diagnostics, breast ultrasound, and breast MRI studies at their facilities. The contractor must be certified by the American College of Radiology for applicable modalities, have a current and valid mammography MQSA Certificate issued by the FDA, or an FDA-approved State certification for mammography. The facility must be accredited by the ARC American College of Surgeons Stereotactic Breast Biopsy for Interventional Stereotactic Breast Procedures. The contractor shall be available for consultation, film interpretation, emergency procedures, from 8:00am to 4:30pm Monday thru Friday. Interested firms are reminded that in accordance with FAR Clause 4.1102 contractors are required to register in the Central Contractor Registration (CCR) database located at http:// www.ccr.gov prior to award of a contract and in conjunction with FAR Clause 4.1201, all prospective contractors shall complete electronic annual representations and certifications at http://orca.bpn.gov. Interested parties responding to this sources sought notice shall submit as part of their capability statement the following information at a minimum (1)company name and address, point of contact with title and a phone number, fax number and e-mail address ;(2) DUN and BRADSTREET number;(3) documentation that its business is composed of health practitioners (MD) primarily engaged in the independent practice of radiology (4)past performance information demonstrating that the contractor has performed radiology services as outlined in this sources sought notice (5)documentation indicating that the Contractor has the required full field digital mammography equipment (6) and any other pertinent company information. The response date to this notice is May 21, 2010 at 4:00PM EDT. Electronic responses are preferable to Linda.Ward2@va.gov. Firms who can meet the requirements stated above must respond, in writing, with information describing their interest, capabilities, and indication if their firm is a small business and which business type/socioeconomic category they qualify.
