Contractor furnished services are to be provided in accordance with (IAW) the Government stated needs at the time of order placement. The service level provides for and requires the Contractor to furnish commercial grade installation services on an as-needed basis. The Period of Performance base year is expected to be from 15 July 2010 through 14 June 2011, with 2 one-year options. The North American Industry Classification System (NAICS) is 238210 and the small business size standard is $14 million. The solicitation/Request for Proposal (RFP) number is FA2823-10-R-0012, and is a 100% Hub Zone set aside. All qualified, certified Hub Zone Small Business contractors are encouraged to participate. Award will be made utilizing Performance/Price Tradeoff procedures with a technical proposal rated on a pass/fail basis. Provisions and clauses are incorporated in this solicitation, and are those in effect through Federal Acquisition Circular 2005-40 effective 22 April 2010. SEE ATTACHMENTS: (A) Solicitation/RFQ FA2823-10-R-0012 and its five (5) attachments: Attachment (A-1) - Performance Work Statement (dated 5 Feb 2010); Attachment (A-2) - Wage Determination 2005-3033, Revision 9, 9 March 2010; Attachment (A-3) - Sample Task Project - Statement of Objectives ((SOO) - dated 18 March 2010); Attachment (A-4) - Past and Present Performance Questionnaire; and Attachment (A-5) - Systems and Wire/Cable Specifications (dated 16 April 2010). Attachment (A-3) Sample Task Project SOO has its own 8 attachments - drawings: Attachments 4.1 through 4.8. A non-mandatory site visit will be held at 0800 (8:00AM) CENTRAL STANDARD TIME on 13 May 2010 at Eglin AFB, FL 32542. Vendors interested in attending the site visit should contact the 96th Contracting Squadron as soon as possible, in order to obtain and submit required background check paperwork/information (Affidavit Forms), necessary for possible attendees to obtain base passes. Required contractor paperwork & actions for base access/passes are described in the following paragraphs. Contracting Office (96th CONS) POC's for site visit information and obtaining the necessary Affidavit Forms are: 1) David Serbe, contracts specialist, at either telephone 850-882-0347 or email david.serbe@eglin.af.mil; and (2) Kristina Mercer, Contracting Officer, at either 850-882-0334 or email kristina.mercer@eglin.af.mil. Both use mailing address 96 CONS/PKBA. 308 West D Ave, Bldg 260, Suite 130, Eglin AFB FL 32542-6862 and fax number 850-882-1680. Site visit attendees will meet Gov. contracting and customer personnel at the Eglin AFB East Gate (Niceville/Valparaiso side of the base) Visitors' center at 0800 (8:00AM) CENTRAL STANDARD TIME on the morning of 13 May 2010. If the required paperwork has been submitted for the attendees, 96th Security Forces at the East Gate Visitors' center will issue base passes for the day of the site visit. Contracting personnel will escort attendees to Range 51, the site of the work in the solicitation's Task Proposal (solicitation Attachment #3). The site visit will take approximately four hours, including travel from main base to the range. Contractor Paperwork & Information Required to Obtain Eglin AFB Passes for the SITE VISIT: (1) the contractor is responsible for obtaining base identification and passes for all contractor personnel who make site visits on Eglin AFB. (2) The contractor shall submit a written request on company letterhead and email/fax/mail the request to the appropriate contracting specialist coordinating the site visit on Eglin AFB (see POC information mentioned previously). The request should contain the names and social security numbers of contractor personnel who plan to attend site visit. The company letter shall be signed by a corporate official authorized to sign such a requests. The Gov. contract specialist will then process the contractor personnel into the Eglin AFB Security Forces data base. (3) Contractors requiring temporary base access for meetings, site visits, etc., must complete the Eglin Contractor Access Badge (CAB) Affidavit Form. When completed, Affidavit Forms must be delivered - either physically or by mail - to the Eglin East Gate Pass & ID Office/Visitors' Center, in sealed envelopes. Multiple Affidavits from the same contractor may be submitted in the same envelope. Envelopes should be addressed to: 96th Security Forces Squadron, Attention: Pass & Registration, 302 West D Ave, Eglin AFB, FL 32542. Envelopes should also have a return Point of Contact (POC) and telephone number on the fronts of the envelopes. Non-local vicinity contractors may MAIL or FED-X the envelopes to the same address listed 2 sentences previously. (4) Anticipate a three-day lead time for Security Forces to process the Affidavits and conduct the required background checks. Once background checks are completed, the Affidavits will be returned to the POC's listed on the envelopes. (5) When reporting to the registration offices at either Eglin AFB gates (East or West) to pick up passes, the authorized contractor individuals should be prepared to present a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and socials security card, in order to obtain a vehicle pass. The solicitation package must be downloaded from Http://www.fbo.gov; no hard copies will be available. This acquisition will be solicited in accordance with FAR Part 12, Acquisition of Commercial items, and FAR SubPart 13.5, Test Program for Certain Commercial Items. All prospective offerors must be registered in the Central Contractor Registration Database (CCR) at www.bpn.gov/ccr prior to the anticipated date of award. Offers/Quotes must be received NO LATER THAN 1600 Hrs. (4:00PM) CENTRAL STANDARD TIME ON 27 MAY 2010.
