- This is the complete copy of RFQ #N00183-10-T-0221. Mandatory Site Visit (Walk-Thru) of Installation Area is Thursday & Friday, 20 & 21 May 2010 at 0900 (9:00 a.m.). If there are any questions &/or to schedule company for site visit, please contact Carolyn Johnson via phone (757-953-5740) or e-mail (adline.johnson@med.navy.mil). This solicitation is restricted (set-aside) to small businesses. Requirement to be awarded on a brand name or equal basis. Quotes should reflect pricing on all items for consideration. Technical review of vendor's quote will consist of SOW (your intent to conform to what is asked of you in the PSOW), presentation of your products, past performance, & price. Site Visits (Walk-Through) of areas of installation will be conducted on Thursday (Boone Dental Clinic) & Friday (Naval Dental Clinic Norfolk), 20 & 21 May 2010 at 0900 (9:00 a.m.). **YOU MUST ATTEND THE SITE VISITS TO QUOTE ON THIS RFQ!** To schedule site visit, please contact Carolyn Johnson, Contracting Division, Materiel Management Department, Naval Medical Center-Portsmouth, VA at adline.johnson@med.navy.mil with the following information for base access: Company's Name, Attendee's FULL Name (FIRST, MIDDLE, LAST), Position Title, Social Security Number, Date of Birth, Place of Birth, & Citizenship (If born outside of the US, please attach the following document: US Pass Port, Naturalization Papers, or Birth Abroad Certificate) This information must be received no later than 1300 (1:00 p.m.) on Tuesday, 18 May 2010. Only contractors who respond timely with the above information will be granted base access & be allowed to participate in the Site Visits. If you are a contractor that has base access & would like to attend the site visit (walk-thru), please contact Carolyn Johnson of your intent of attendance. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). If freight charges apply, indicate if charges will be "per shipment" or "single one-time charge". Contractor shall supply a current published price list or GSA/VA Federal Supply Schedule contract for supplies &/or services being procured. Solicitation closes on Friday, 28 May 2010. STATEMENT OF WORK FOR FURNITURE AND CABINETRY FOR SEWELLS POINT & BOONE CLINIC DENTAL DEPARTMENTS General Requirements • Vendor to provide furniture & cabinetry for the Dental Clinics at Naval Dental Clinic-Norfolk, 1647 Taussig Blvd., Norfolk, VA & Boone Branch Dental Clinic, 1035 Nider Blvd., Suite 100, Naval Amphibious Base-Little Creek, Norfolk, VA. • Vendors price to be inclusive of all cost including material, labor, shipping, storage installation and trade-in (removal & disposal) of furniture design of layout. • Storage space will be limited to actual worksite. Vendor will be required to store all materials. • All removed furniture shall become the property of the vendor. • All storage / file area are to be able to hold a minimum of 70 lbs of weight (factor for 3' x 1' storage area). • Vendor shall not cover up existing power locations. • Installation and of furniture shall be scheduled in a manner that minimizes the impact on the Government. This requirement is a time critical project. Furniture / cabinets must be installed as soon as possible after award. • All installed / supplied items are to incorporate to the maximum extent possible to be efficient in design with an emphasis on minimizing excess movement of employees during daily task and incorporating the maximum amount of work and storage space possible. • All installed / supplied items are to be designed with the ability to easily clean. Vendor to contact end user as soon as possible after receipt of award to select product finishes. • Replacement parts for supplied items must be made available for a minimum of 5 years • The Government shall have the right to approve all furniture and fabric colors. • The selected vendor is encouraged to correct punch list items as soon as possible. Note that final payment will be held until all punch list items are corrected. • Vendor will be required to perform all work required for this project including making required electrical and plumbing connections (as they apply). • The vendor will be required to verify building structure prior to attaching furniture to any walls, floors or ceilings. Sizes of furniture items are listed as minimum sizes. Vendor may increase sizes as required but should not offer smaller products unless approved by the Government.
