- This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6 and Far Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through the most recent Federal Acquisition Circular. The NAICS Code is 336413. The Department of Hunter Holmes McGuire Veterans Affairs Medical Center, Richmond, VA has a requirement for scopes and components and intend to solicit quotes and award a firm fixed price contract. This is a Total Small Business Set Aside. Contractor shall provide the following equipment/supplies/Service: (1) ENT 5000 Flexible Laryngoscope Item number 02-5201, Qty 08 EA. (2) Digital Video Processor W. LCD and air Insufflation (DPU-5050A), Item number 07-5051, Qty 04 EA. (3) TNE 5000 Flexible Video Esophagoscope, Item number 03-5201, Qty 02 EA. (4) Mobile Endoscopy Cart, Office, Item number 07-6140, Qty 04 EA. (5) Stroboscopy System STR-5000, Item number 07-5070, Qty 01EA. (6) TV-2.1 Endosheath System, Item number 03-5101, Qty 10 EA. (7) TV-1.5 Endosheath System, Item number 03-5102, Qty 10 EA. (8) ENT Slide-On Endosheath System, Item number, 22-5601, Qty 20 EA. (9) ENT Channeled Endosheath System, Item number 44-5601, Qty 10 EA. (10) V1 Installation Stand- Floor Model, Item number 07-6160, Qty 04. (11) MFG Instruction for cleaning and reprocessing, Qty 2 EA. The following provisions apply to this solicitation: 52.211-6 Brand Name or Equal, 52.212-1, Instructions to Offerors-Commercial, 52.212-4, Contract Terms and Conditions-Commercial, Commercial Advertising, 852.271-70, 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial, including clauses incorporated by reference under Section b (1)(5)(11-16)(23-24); Offerors are to submit pricing. Process of determining contract award: award will be to the lowest-priced responsive, responsible bidder who can provide the items. Warrant information shall be included in the proposal. All questions are to be submitted via email, response to questions received after May 20th 2010 will not be answered. All proposals must be submitted in written to the email address provided. This information should be specific, concise, and complete and provide sufficient information to demonstrate the offeror's capacity to meet or exceed the requirements of the contract.
