- Amendment 0003 to statement of work. Amendment 0003 to RFQ FA5205-10-Q-KL29. Change statement of work and change Contracting Office POC. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation (Request for Quotation) reference number is FA5205-10-Q-KL29. The solicitation incorporates provisions and clauses as those in effect through Federal Acquisition Circular 05-38. This procurement is NOT a set-aside. The North American Industry Classification System code (NAICS) is 423910 sporting and recreational goods and supplies merchant wholesalers. Offerors are required to forward proposal and technical documentation by 4:30 am JST 30 April 2010. DESCRIPTION: Installation service for playground equipment and tiles at Weasels' Den bldg 973, Misawa AB. Please refer to the attached RFQ No FA5205-10-Q-KL29. PROVISIONS/CLAUSES: The following FAR and DFARS provisions and clauses apply: 52.222-3, 52.222-21, 52.222-26,52.222-36, 52.225-13, 52.232-33, 52.228-3, 52.228-4, 252.232-7010, 252.232-7003, http://farsite.hill.af.mil.
