Description
This is a combined synopsis/solicitation for site clean-up and debris removal services prepared and processed in accordance with the Federal Acquisition Regulations (FAR) Part 12.6 as supplemented with additional information included in this announcement for the Department of Interior (DOI), Bureau of Indian Affairs (BIA), Western Region. The DOI/BIA/Western Region, requires a firm fixed-price contract. This notice is hereby issued as RFQ. No. RMH00100059. This constitutes the only Request for Quote (RFQ) that will be issued and a separate RFQ will NOT BE ISSUED. All responsible sources are encouraged to submit a quote. Quotes shall be submitted to Santiago Almaraz, 2600 N. Central Avenue, Suite 450, Phoenix, AZ 85004 or emailed to santiago.almaraz@bia.gov by close of business 4:30 PM (Local Time) on Tuesday, June 15, 2010. This is a 100% set-aside for Total Small Business. The NAICS code for this acquisition is 561730. The Small Business Administration size standard is $7.0 million. 1. SCOPE OF WORKBureau of Indian Affairs - Western Region - Pima AgencyDebris Removal and Site Clean-up (Environmental) Location: Gila River Indian Community (GRIC), location near Laveen, AZ (See map attachment) Period of Performance: Seven (7) days completion from identified project start date. Purpose: Environmental Clean-Up (Landscape debris only) on the Gila River Indian Community. Contract shall remove all piles of landscape debris only, from the following property: Allotments 4348 and 4345 located in Section 12, Township 1 South, Range 1 East, on the Gila River Indian Community (GRIC), 75th Avenue and Elliott. Clean-up is to eliminate a community fire hazard from external debris dumping. Estimated debris area is 2 acres. Email santiago.almaraz@bia.gov to request an electronic copy via email of the site maps available for this project. There are no known hazardous materials within the landscape debris. Quote for debris clean-up and disposal work shall include the following:-All application and permit fees.-All related disposal fees-Labor required for debris clean-up work and equipment operation-All equipment and materials, as required to complete job, i.e. loader, tractor, water truck.-All other related fees. All work to be completed in a workmanlike manner according to standard practices. Site will be monitored by one or more designated individuals from the Bureau of Indian Affairs (BIA) and/or the GRIC. Monitors will determine when site clean-up is complete. Site clean-up shall be accomplished with minimal soil disturbance. There are specific designated areas for equipment operation during the clean-up process. A complete list of employees and equipment utilized at the site shall be submitted to the BIA prior to obtaining permits and licenses. Contractor shall comply with all BIA and GRIC laws and ordinances. Contractor shall contact Gila River Indian Community to identify any and all associated tribal permit fees required to complete job. Proof of permits are not required at time of quote submission but proof will be required before work is to commence. 2. FINAL INSPECTION All work will be inspected by the Government to verify that it meets the requirements of this contract. Acceptance will be made upon the Government's satisfactory inspection. 3. CONTRACT CLAUSESThis notice is hereby issued as RFQ No. RMH00100059. This RFQ will be conducted as a commercial service acquisition in accordance with FAR Part 12. The Department of Labor Service Contract Act, Wage Determination No. 2005-2023, Revision 11, dated 8/15/2009 is applicable to this requirement. The following provisions and clauses are incorporated and in effect through Federal Acquisition Circular (FAC) 2005-26. The FAR clauses and provisions are incorporated into this acquisition shall be: 52.212-1 Instruction to Offerors- Commercial Items (Jun. 2008), 52.212-3 Offeror Representations and Certifications - Commercial Items (Jun. 2008). 52.212-4 Contract Terms and Conditions - Commercial Items (Feb. 2007), 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (Jun. 2008) (Sections 5, 15, 16, 17, 18, 19, 21, 24, 28, 31, 36), (c)(1). FAR clauses are available on the Internet Website http://www.arnet.gov/far. The following Commerce Acquisition Regulations (CAR) clauses and provisions are also incorporated into this acquisition by reference: 1352.201-70 Contracting Officer's Authority (Feb. 2005), 1352-201-71 Contracting Officer's Technical Representative (COTR) (Feb 2005) there will be a government representative designated at time of award, 1352.215-77 Evaluation Utilizing Simplified Acquisition.
