Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued This solicitation number is NHLBI-PB-(HL)-2010-185-DDC and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-41 (5//2010). The total contracted dollar amount, including options, if any will not exceed $5 million. The acquisition is being conducted as a full and open competitive, with no set aside restrictions. The North American Classification System (NAICS) code applicable to this requirement is 334516 and the associated small business size standard is 500 employees. The delivery point is the National Institutes of Health, National Heart, Lung, and Blood Institute (NHLBI), Bethesda, Maryland. The Government=s anticipated delivery date is to provide a standing order for a period of six months after receipt of purchase order. Market research conducted by the Government in the FedBizOpps for fifteen days, and there were no responses to the Source Sought. The National Institutes of Health (NIH), National Heart, Lung, and Blood Institute (NHLBI), Office of Acquisition (OA), intends to award a single, fixed priced purchase order for the specification listed below. Major tasks required of the Contractor include the following: 1.Catalog #GD-103-4010, 10 Single-Read Cluster Generation Kit v4, ship one (1) kit once a month starting upon receipt of purchase order, for the period of six (6) months. 2.Catalog #FC-104-4002, 36 Cycle Sequencing Kit v4, ship 10 Kits. FAR Provisions apply to this acquisition are: FAR Clause 52.212-1 Instructions to Offerors Commercial Items (JUN 2008) 2) As stated in FAR Clause 52.212-2 (a), The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: The purchase order award will be made to the Offeror whose Part meets the Government=s minimum performance specifications, and delivery date. The Offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The addendum to FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, is as follows: The Offeror must include in its quote, the unit price, the list price, shipping and handling cost, the delivery date, prompt payment discount terms, the F.O.B. point (Destination or Origin), and provide descriptive literature inclusive of specifications as part of the quotation. The DUN and Bradstreet Number (DUNs), the Taxpayer Identification Number (TIN), and the business size. The Offeror must be registered in the Government=s Central Contractor Registry (CCR) System, which is available at www.ccr.gov. The FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - apply to this acquisition.
