Description

Subcontractor Construction Safety Qualification Form (SQF) SLAC National Accelerator Laboratory is currently looking for additional sources who are licensed contractors that have experience modernizing/ upgrading/and repairing different types of elevators manufactured by various companies that range in age from about 35 years old to almost new. Please reply with a company capabilities statement, list of relevant projects, and points of contact including telephone number and Email address. SLAC requires all contractors to submit a copy of their OSHA compliant Injury and Illness prevention Plan/Program and to complete a Safety Qualification form that must meet SLAC requirements to be approved to work on site. The qualification form is attached and you are encouraged to complete it to the best of your ability and return a completed form with any reply or response to this posting. I prefer to receive Emails; however, may be available to speak with you if desired. Your interest in doing business with SLAC is appreciated. SLAC National Accelerator Laboratory is operated by Stanford University for the Department of Energy and as such several Federal Acquisition Regulations apply to all procurements, including prevailing wage determinations.
