STATEMENT OF WORK/DESCRIPTION OF SERVICES 1.1 CONTINUED SUPPORT: Critical Care (ICU, OR, and PAUC), Clinical Data Stores, and Healthcare Analytics System. A hospital, building a data warehouse comprised of data from its clinical systems, should expect to be able to answer questions addressing clinical quality, medical error reduction, healthcare costs, and differentiating quality, as well as a wide range of clinical research endeavors. However, commercially available healthcare data warehousing activities relate primarily to (1) the storage and preservation of data, and (2) providing answers to known questions, either through standard reports or structured ad hoc queries. These systems are insufficient for analyzing clinical information outside of highly structured and limited domains. In analyzing clinical data, new knowledge is most often discovered by finding new questions, adding new data elements without first modifying the model, and working freely across domains limited only by the availability of data. An effective healthcare analytics solution will then allow physician and other clinicians to leverage the value of clinical data stores to decrease medical errors, and improve the quality of veteran health care. The Healthcare Analytics Solution is a powerful analytical environment which provides healthcare professionals with the ability to directly access clinical data stores, thus enabling improved patient care, a reduction in medical errors, and the conduct of a broad range of research initiatives. To accomplish these goals, the Healthcare Analytics Solution allows clinical users to directly query data generated by diverse systems. The Healthcare Analytics Solution supports rapid, iterative analysis of clinical data by domain knowledgeable people through an intuitive user interface including the analysis of temporal relationships. New queries or algorithms can be deployed without first requiring support from information technology experts. The Healthcare Analytics Solution is a secure web application that provides a graphical user interface to the desktop of approved users. From their browser based interface, the user then queries Healthcare Analytics Solution analytic data stores created from a variety of sources. Users may then select data across tables (for example systolic blood pressure, serum potassium, surgical procedure, operating time, and mortality) and then form an analytical query. The Healthcare Analytics Solution provides the ability to create an analytic table, run an analysis, evaluate the results and then iteratively make adjustments both to the analytical tables and algorithms on the fly. The goal of the Healthcare Analytics Solution is to provide clinicians with a tool that will help them improve the quality of care delivered to veterans. The following bullets highlight the general work flow a user would follow: 'Identify a collection of clinical data that is of interest. The user can add new data elements without having to first modify the model, and may work freely across domains limited only by the availability of data. 'Create a general or specific query against the identified clinical data. 'Analyze data sequentially (work across rows) and perform time series analysis to answer clinical questions. 'Aggregate the table, and create summary statistics. 'Examine and evaluate the results of the query. 'Modify and re-run the query. 'Graph results or export to a statistical analysis package. 'Perform iterative analysis without an intermediary (IT person). 'Develop and deploy parameter driven standard reports from the GUI. The initial implementation includes a library of standard reports, populations, and analytic algorithms. 1.2 Server Administration 'Issue network operator commands, display, start, stop, re-start and reset devices 'Assume the responsibility for and perform all console operations, including, monitoring, alarm systems and environmental controls, and transmission and reception of polling information from outside organizations 'On an ongoing basis, enhance processing capabilities and efficiencies through system tuning and other run time improvements. Perform continuous monitoring of utilization needs and efficiencies and report on tuning initiatives monthly 'Perform proactive failure trend analysis 'Perform weekly, as scheduled with, as identified in the Service Level Agreement, emergency systems maintenance 'Maintain and update the operational documentation for all operations procedures and services. Update documentation within 10 calendar days of a change occurring; 'Analyze midrange performance metrics 'Perform regular direct access storage backups on all systems 1.3 Systems Administration 'Perform Operating System ID assignment 'Constantly monitor system operations 'Install, maintain, and provide ongoing support for Software 'Enforce documentation standards 'Report resource utilization statistics related to Systems Software release level upgrades; 'Provide technical advice and support to the Business Applications and DBA staffs, as required 'Perform activities required to monitor and to provide performance monitoring, tuning, capacity planning and configuration to optimize performance to reduce costs and/or improve service levels 'Conduct routine monitoring via the use of Software tools to measure the efficiency of DASD access and take corrective action including performance adjustments to Equipment, Software, or file placement as required to maximize service 'Backup and Restore 'Perform routine backup (both full and incremental) and recovery procedures in accordance with the requirements of applications or of the Business Continuity Plan and so as not to adversely impact scheduled operations, including regular DASD backups on all systems. Utilize automated backup procedures wherever appropriate 'Disaster Recovery 1.4 Disaster Recovery of the analytics database 'Plan for disaster recovery of the analytics database 'Perform incremental and full backups 'Coordinate off-site storage of backup media with the VISN 'Recover the analytics database after a disaster 1.5 Physical Database Administration 'Periodic updates of data from clinical systems 'Storage management services 'Installation, maintenance, performance tuning, upgrades, restarts, tailoring, monitoring and necessary upgrades of the database software products 'Backup and recovery standards and support for the databases 'Data modeling of business requirements into database objects 'Perform physical database management system ('DBMS') database control functions to support systems existing as of the Effective Date and planned new systems development 'Plan for changes in the size of databases due to business growth, or decline, and project implementation based on information supplied by VISN 21 and review plans with VISN 21 on a regular basis for VISN 21's comment and approval 'Monitor database performance (includes automated monitoring tools) 'Perform reorganizations when required to optimize performance 'Implement database security / privilege processes and procedures 'Test and implement database archive processes and procedures 'Recovery from an outage or corruption in a timely manner 'Capacity planning for maximum efficiency of hardware and software 'Database performance analysis 'Provide technical advice to the application development and DBA staffs and assist applications development and maintenance staff in performing stress testing and operating system and database performance tuning 1.6 Administration and support of the analytic environment Solution Analytics and RGI will provide support to both clinical and non-clinical users of the analytic environment, including but not limited to the following: 'Monitor performance of analytic interrogation engine and analytic data stores 'Maintain and upgrade analytic engine and analytic data stores 'Upgrades of the analytic interrogation engine and analytic data stores 'Upgrades to the browser based graphical user interface 'Upgrade system event/query library 1.7 User Administration VISN 21 may administer user privileges and access to the database from the analytics GUI. A VISN 21 analytics administrator may add new users to the system. The VISN 21 administrator can then assign the user a level of access to the analytic features, and specify which data a user may access. The analytics administrator can create a user group and then specify the data to which that group has access. The analytics administrator may then assign users to one or more groups. The analytics administrator will also manage the private sharing of query parameters and other sharable work. 1.8 Security Co-located in a VISN 21 data center, the analytics server will be protected by the same level of physical security as any VISN 21 server. The server will reside on the VHA network and within the VA firewall conforming to all VA network security requirements. Solution Analytics will ensure browser access is through 'https' and that each user is assigned a username and password conforming to VHA requirements. Solution Analytics will also ensure that the Healthcare Analytics Platform, working through database security features such as fine grain access control, allows the VISN 21 administrator to define user access to data at the level of hospital, row, column, and table. 2.0 CIS Server database maintenance support. This service includes on-going support and maintenance of: 'OR/ICU Critical care PICIS servers (most server issues can be handled remotely by our DBA / server administrator in Palo Alto, but physical administration of hospital servers other than Palo Alto would be accomplished by on-site VISN 21 IT personnel under the guidance of our DBA/server administrator) 'Operating Room (OR) PICIS servers, including conscious sedation data (once fully implemented and in production mode) 'Central VISN server 'One way replication of the Palo Alto PICIS database to the other VISN hospitals 'PICIS and VistA extracts to the analytics server, including OR and conscious sedation data. 'On-going support to automate new standard tables and other database customization for VISN 21 custom reports and populations, including OR and conscious sedation data. 'Staging, and load of new types of data to be exported from VistA, as well as the management of the new VistA extracts. 'Re-staging of data related to the new extract data software. 'On-site experienced Oracle DBA / system administrator at 0.4 FTE 3.0 Custom analytics provider and QI reports training. This section addresses additional training of VISN 21 personnel in the use of the analytic environment to create and maintain quality improvement/resource utilization reports, as well as the creation of custom provider reports, and custom enhancements to the user interface. Custom user inter-face enhancements: This service includes implementation and customization of GUI based access to data from the ICU, operating room, PACU, and conscious sedation areas with respect to: 'Information regarding the data available for analysis 'Analysis 'wizards' to facilitate table, query and result creation, and 'Field analysis and manipulation tools, including conversion of dates (date stored as number <=> date, date=>month, year, dow, hour), numbers stored as strings to numeric storage, and continuous numeric values to categorical with user controlled 'bin' limits Custom reports: This service includes support for the implementation and customization of VISN 21 reports. This will include the customization of at least four report templates that allow reports to be generated on multiple levels - VISN to provider. At least four individual reports will be created using the templates oriented to important clinical areas or clinical issues. The VISN will be responsible for selecting the four areas of focus for this activity, and providing a prioritized list of initial measures for each selected area. The reports will include features that allow for the user to 'drill down,' access additional data, and analytic features. Documentation for the use of these templates will be provided. In addition, this will in addition include both the production of new quality improvement, department director, and provider report templates and related populations as well as the adaptation / modification of templates and tables created for intensive care data. The methodology associated with the 'bundle' report templates and tables produced to support VISN 21's participation in the VA IPEC program best illustrates this service. An initial effort produced report templates for the ICU 'bundle' program, and supported VISN21 in their implementation and use. VISN 21 staff now independently use the bundle reports to monitor bundle implementation, performance, and clinical outcomes. This model has effectively been extended to reports supporting quality improvement efforts to prevent skin breakdown (Braden and wound reports), and decreasing the unnecessary use of indwelling foley catheters. Custom Analytics activities will also include support for VA Data standardization initiatives and basic user support. 4.0 Extension of the analytics license and the addition of OR data. This service includes extension of the analytic license to all VISN 21 patient related data including OR clinical information system, as well as comparative analysis to other VA data from outside the VISN or to any other benchmark data. 5.0 PICIS SERVER ADMINISTRATION: The contractor shall work with VA personnel and the Analytics Database Administrator (DBA) to perform the following services in support of the PICIS Servers: Install operating system on new PICIS servers. Configure the servers for the right partitioning of system and database. Work with VA network personnel to configure the servers on the VISN-21 network. Install/monitor the production servers in each facility: Palo Alto; Northern Cal; San Francisco; Reno; and Fresno (other sites in VISN 21 may be added in the future). Install standby servers at the Palo Alto facility. Work with the Analytics DBA to configure the Oracle failover system using Oracle Data Guard. This includes both system initiation and testing to ensure the failover system is working properly. Work with the Analytics DBA and facility clinical and technical point of contacts (POCs) to coordinate and perform database shutdown activities during maintenance activities. Configure backup software, perform routine system backups, and test recovery procedures. Install security patches and anti-virus applications. Participate in system integration testing at each site. Other services as required to support the PICIS system. 6.0 PICIS CLIENT WORKSTATION SUPPORT: The contractor shall work with Veteran Affairs (VA) personnel to perform the following services against the PICIS client workstations. As this support requires familiarization with PICIS and medical monitoring equipment, in addition to system administration expertise.VA personnel will train the contractor on PICIS and medical monitoring equipment specifics. With this training, the contractor shall either directly or working in support of local on-site VA personnel perform the following as required: install PICIS clients on bedside and remote view workstations, troubleshoot and determine problems between the Application and medical monitoring equipment, install application upgrades (PICIS service packs) and coordinate to other VISN sites POCs. Troubleshoot, analyze, and read HL7 messages from VistA. Troubleshoot PICIS application device drivers. Determine VistA-PICIS security keys to assign for new users. Troubleshoot serial interface between the workstations and medical monitoring equipment. If PICIS corporate technical support is required to resolve any technical issue, initiate support requests as appropriate on behalf of the local support team. Provide other services as required to support the PICIS system. 7.0 HOURS OF COVERAGE: Normal work hours: The contractor's normal tour of duty shall be from Monday to Friday from 8:00am to 4:30pm Pacific Standard Time (PST) and shall be performed on-site at a mutually agreeable VISN 21 facility. Non-duty hours: The contractor shall be available during non-duty hours to provide off-site emergency support to the VISN 21 PICIS system and local support staff and therefore will be on-call 24/7. In order to provide 24/7 support, the contractor shall maintain a contact list of all remote clinical and technical POCs for VISN 21. Occasional hours: will be worked outside the normal tour of duty when installs, upgrades, etc. are performed to minimize the impact and risk relative to users/customers of PICIS system. Travel: Occasional travel will be required throughout sites within VISN 21 to install, test, support, and train key VA staff on technical aspects of the PICIS system. 8.0 COMPLIANCE AND BUSINESS INTEGRITY (CBI) TRAINING 1. AWARENESS TRAINING: Contractor employees shall complete initial compliance awareness training within thirty (30) days of commencing work under this contract as well as complete annual compliance awareness refresher training. At the minimum, CBI awareness training will include the following topics: 1) the revenue cycle, 2) seven elements of an effective compliance program, 3) definition of high risk areas, and 4) definition of a compliance concerns and how to address a compliance concern. This requirement can be fulfilled by completing the training module available via the following Internet site: http://www.visn21.va.gov/CBI.asp. 2. REMEDIAL TRAINING: When notified, contract employees must complete remedial training and education to address any detected compliance exceptions. 3. PROOF OF TRAINING: Contract employees are responsible for submitting proof of awareness and remedial training completed to the Contracting Officer's Technical Representative (COTR) for this contract. The COTR will retain proof of training in accordance with applicable Records Control Schedule. 9.0 CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: Upon contract award, all key personnel shall be subject to the appropriate type of background investigation or screening per VA/VHA policy as delineated below, and must receive a favorable adjudication from the local VA facility or VA Security and Investigations Center (SIC) depending on the type of investigation/screening required. This requirement is also applicable to all subcontract personnel. If the investigation or screening is not completed prior to the start date of the contract, the Contractor will be responsible for the actions of those individuals they provide to perform work for VA. Contract personnel who previously received a favorable adjudication as a result of a Government background investigation or screening may be exempt from this contract requirement provided that they can provide documentation to support the previous adjudication. Proof of previous adjudication must be submitted by the Contractor to the VA Contracting Officer. Proof of previous adjudication is subject to verification. Some positions maybe subject to periodic re-investigation/screening. 1.Position Risk/Sensitivity - For all positions required under this contract, the position risk/sensitivity has been designated as: Low Risk/Non-Sensitive 2.Background Investigation/Screening - Since it is anticipated that the Contractor or contract personnel will be providing non-health care related services at a VA facility(s) under a single contract or series of contracts and WILL NOT have access to VA information systems, the background investigation/screening commensurate with the requirements of this contract is: Background Screening - Includes Fingerprinting, Verification of Identity, and Credentials/Employment History, if Necessary. 3.Contractor Responsibilities a.The Contractor shall prescreen all personnel to ensure they are able to read, write, speak, and understand the English language. b. Within five (5) business days of contract award, the Contractor shall submit or have their contract personnel provide any information or documentation required to conduct the background screening as requested by the local VA facility or VA Contracting Officer. c.Once the items requested in paragraph 3. b. above are received, VA will pre-screen these items for completeness, and forward them to the appropriate party(s) in order to initiate the required background investigation(s) or screening(s) within fourteen (14) calendar days of appointment. Only after the VA Contracting Officer notifies the Contractor that the background investigation(s) or screening(s) was initiated shall the Contractor be authorized to provide services under the contract. As previously stated, if the investigation or screening is not completed prior to the start date of the contract, the Contractor will be responsible for the actions of those individuals they provide to perform work for VA. d.The Contractor, when notified of an unfavorable determination by the Government, shall withdraw the contract person from consideration of working under the contract. e.Failure to comply with these Contractor personnel security requirements may result in termination of the contract for default. 4.Government Responsibilities a.The Contracting Officer's Technical Representative (COTR) will be responsible for insuring the Contractor personnel security requirements described herein are fulfilled. b.Upon receipt, the local VA facility or VA SIC, depending on the type of investigation/screening required, will review the accuracy of the items requested in paragraph 3. b. above, and forward these items to OPM to conduct their portion of the background investigation or screening, as applicable. c.The requesting VA facility will pay for any portion of the investigation or screening conducted by OPM, if any. d.Depending on the type of investigation/screening required, the local VA facility or VA SIC will notify the VA Contracting Officer after adjudicating the results of the background investigation or screening. e.The COTR will ensure that the required investigations or screening have been completed or are in the process of being requested. Personnel Identity Verification (PIV) of Contractor Personnel In accordance with FAR 52.204-9 and VA Directive 0735 - Personal Identity Verification of Federal Employees and Contractors, any contract person who requires routine physical access to a Federally-controlled facility and/or routine access to a Federally-controlled information system will be required to verify their identity prior to providing services under the contract. Prior to providing services under the contract, each contract person will be asked to provide up to two (2) forms of identification from the Accepted Identification Documentation List to the appropriate VA representative in order to obtain a proper VA-issued identification card. See the Accepted Identification Documentation List provided below. The COTR, or designee, will be responsible for sponsoring each contract person that requires a VA-issued identification card. PIV ID Proofing Criteria The following criteria must be met by all VA employees, contractors, and affiliates prior to being issued a PIV card or Temporary Identity Badge. Table of Accepted Identification (From Form I-9) Last Update: June 5, 2006 1. Two forms of identification are required from the list of acceptable documents. At least one ID must be a state or federal government issued picture ID. Either of the following is accepted: a. Two forms of identification from the left column (Federal or State Government issued picture ID). b. One form of identification from the left column (Federal or State Government issued picture ID) and one form from the right column (NonPicture ID or Acceptable Picture ID not issued by Federal or State Government). 2. Any form of identification used for ID proofing may not be expired. 3. Hand written or photocopied documents are not accepted. 4. An ID issued before a legal name change (e.g. birth certificate or driver's license) can be presented as one form of ID if a legal document (e.g. marriage certificate/license or a court order) is also presented linking the previous name to the current legal name. The linking document has to display both the former and current legal names. Both documents must be valid and not expired. For example, a married woman may use both a certified copy of her birth certificate and marriage license as one form of ID as long as the marriage license has to display both her maiden name and married name. 5. The Applicant's name listed on the VA Form 0711, Request for One-VA Identification Card, must match the name on one of the IDs presented by the Applicant.
