Description
Performance Based SOW This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) # 246-10-Q-0038 and is unrestricted. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44, effective July 8, 2010. The associated NAICS code is 524292, with a standard business size of $7.0 M. Contractor shall provide a firm, fixed-price unit price (all inclusive) for Outpatient Coding and Data Entry Services for August 2, 2010 to September 30, 2010. BACKGROUND: The Indian Health Service has an urgent need for outpatient coding and data entry. STATEMENT OF WORK: The performance based statement of work is attached. EVALUATION FACTORS The provisions of 52.212-2, Evaluation - Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award from this solicitation to the responsible offeror whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. Cost/Price: The following factors shall be used to evaluate offers: The significance of cost/price in the award decision will be 30% and technical will be 70%. Outpatient Coding and Data Entry Services: 20,000 each @ $_____ = $________ Technical: The Contractor must currently have access to the Indian Health Service RPMS system. This is an urgent requirement and services must be completed NLT September 30, 2010. Quotes shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (Aug 2009) or indicate certifications in ORCA at http://orca.bpn.gov. The provisions of 52.212-1, Instructions to Offerors-Commercial Items (June 2008), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) verification of indemnification (2) technical quote; (3) Dun and Bradstreet Number (note: Contractors must be registered in the Central Contractor Registration to be eligible for award.
