- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-69D-10-RQ-0424, is issued as a request for quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-38 effective February 1, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 333312, Size Standard: 500 Employees. This procurement is a small business set aside. Offeror's quotes are hereby being requested and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. Offeror shall provide complete system (furnish and install). The following items are offered by Gardner Machinery Corp. and are known to meet the VA's needs. Offeror shall provide complete system. Offeror shall provide the following Brand Name or Equal items: Description: Item 1: BRAND NAME OR EQUAL to the Gardner Electronic Weigh Cart Model WC 300. 15 Each. According the below listed Salient Characteristics. Price per cart $___________ Discount (if applicable): $___________ TOTAL PRICE FOR ALL 15 CARTS $____________; Total Price shall include all equipment delivery charges. Salient Characteristics: 1) Integrated electronic weight sensing capability; 2) System must have ability to hold up to 150#); 3) Required dimensions 35 ½" X 39 ½" X 56" or a slight variance in measurement is acceptable if it still meats the ability to withhold 150 pounds and meets the rest of the requirements. 4) Calibration settings that can be adjusted for variations in gravity for different locations 5) Low battery indicator 6) LCD display 7) auto shut off with zero display 8) battery powered 9) basket constructed by tubular steel base frame 10) ability to change between decimal, pound or kilogram 11) The cart bag needs to be of a rugged type fabric construction with the ability to withhold 150 pounds of laundry 12) A 1 year warranty will accompany all products Delivery shall be FOB DESTINATION to the WILLIAM S MIDDLETON VETERANS MEMORIAL HOSPITAL, 2500 OVERLOOK TERRACE, MADISON, WI 53705 Attention: Jamie Peters. Offer must include product literature, product samples (if requested), technical features, warranty provisions, and any training and or installation that will be required. Award will be made to the offeror representing the LOWEST PRICE TECHNICALLY ACCEPTABLE to the Government. All quotes will be evaluated in accordance with technical capability, compliance with the solicitation requirements and price.
