Description
The Department of Veterans Affairs, Iowa City VA Medical Center, Iowa City, IA intends to negotiate a sole source procurement under the authority of FAR 6.302.1, with Healthcare Quality Initiative, 28 Ballard ST, Newton, MA 02459. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-263-10-RQ-0390 and issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-35, effective July 14, 2009. The associated NAICS code is 541611 and small business size standard is 7.0 M. The acquisition is not set-aside and is being negotiated as a sole source procurement. The requirement for this acquisition includes the following: ITEMEST. QTY UNIT UNIT COSTEST. TOTAL COST 1.1 Statement of Work: Provide service in the areas of Patient Centered Medical Home (PCMH) benchmarking and program evaluation for the VISN 23 PCMH Demonstration Lab. Tasks to be completed by Dr. Salem-Schatz are as follows: 1. Conduct a literature review of published evaluations and other reports of PCMH implementation. 2. Analyze findings from literature review. Tasks to be completed by Dr. Salem-Schatz in partnership with Jan A. Taylor, Ed.D. 1. Review VHA PCMH materials to develop an analytic framework for benchmarking and interviews. 2. Combine findings from literature review and expert interviews and present in a results table. 3. Plan, conduct and analyze site visit to Grand Island VA. 4. Participate in preparation of final harvest report. Timeline and payment: Contract to run from award thru March 31, 2011 Estimated days:32.
