- Fillable Pricing Schedule Attachments Solicitation Standard Form 1449 Cover Letter This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the Federal Acquisition Regulations (FAR) Part 12 and Part 13, as supplemented with additional information included in this notice. The Social Security Administration (SSA) in Region 10 has a requirement for contractors to provide medical consultant services in a variety of medical specialties. SSA's Center for Disability utilizes the services of physicians in review of disability claims. Services will be performed on site at SSA Region 10 Seattle office. These consultant services will include providing oral and/or written consultations resulting from reviews of SSA disability case files, consulting with SSA medical policy staff and/or training other medical consultant contractors. No patient contact will be involved. New medical consultants (those not currently under contract to provide these services to the SSA) will be required to participate in an orientation program which is intended to enable the contractor to attain and achieve SSA's performance expectations. The BPA's period of performance is approximately January 1, 2011 or date of award, whichever is later, through December 31, 2015. The government intends to award multiple Blanket Purchase Agreements (BPA) for m edical consultant services as a result of this solicitation. The Solicitation Number is SSA-RFQ-11-1005. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-40. The associated North American Industrial Classification Code for Physicians Offices (except Mental Health) is 621111, with a small business standard of $10 million and for Physicians Offices (Mental Health) 62112, small business standard $10 million. This requirement is a 100% Small Business Set Aside. Offers received from other than small business concerns will not be considered. This acquisition is being conducted in accordance with the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures. The selected offeror must comply with the following commercial item terms and conditions as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. In addition, offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with its proposal. Applicable terms and conditions, and representations and certifications are attached. The Government will award BPAs resulting from this solicitation to the responsible offeror whose offer conforms to the requirements of this solicitation To be eligible for an award, Offerors must be registered in the Central Contractor Registration (CCR) database. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. All awardees will be required to undergo a security background screening which will include completion of fingerprint cards before beginning performance under the contract Quotations in response to this combined synopsis/solicitation are due no later than 2:00pm PT on September 9, 2010. All questions regarding this RFQ may only be submitted by email to Linda.Shaw@ssa.gov no later than noon PT on August 25, 2010. E-mail communications shall include the offeror's name, point of contact, e-mail address, and phone number. Questions and their respective answers will be made available to all prospective offerors in the form of solicitation amendments. Pre-proposal conferences are scheduled for August 17, 2010 at 3:00pm and August 18, 2010 at 9:00am at SSA Seattle Office. The information planned for presentation will be the same at both meetings. Questions and their respective answers generated during the pre-proposal conferences will be made available to all prospective offerors in the form of solicitation amendments.
