Description
Program Manager Provider Performance Work Statement (PWS) 1. BACKGROUND Pre/Post Deployment is required for all personnel that are deploying to DOD designated areas. The Pre/Post Deployment will consist of ensuring all individuals are medical qualified and will received needed medical follow assessments upon return. The Post-Deployment Health Reassessment (PDHRA) DD Form 2900 is a Department of Defense (DoD)-mandated commander's program that was developed in compliance with the Assistant Secretary of Defense for Health Affairs Memorandum, Post-Deployment Health Reassessment, 10 March 2005. This memorandum mandates that the Military Services administer the PDHRA assessment (DD Form 2900) to redeployed personnel within 90-180 days of return from deployment to identify and to address health concerns that may have emerged since their return. 2. SUMMARY OF SERVICES REQUIRED This PWS identifies contract requirements for one full-time equivalent healthcare provider to manage operations of the Active Duty USAF PRE/POST/PDHRA programs at Installation. 3. PERSONNEL QUALIFICATIONS 3.1 Advanced degree in accepted field of study from an accredited institution, along with current, valid license and/or certification (as applicable) to practice in field of expertise. Examples include: 3.1.1 Physician Assistant (PA). PA must have a Master of Physician Assistant Studies degree from a PA educational program accredited by Accreditation Review Commission for the Physician Assistant. Candidate also must hold current certification from the National Commission on Certification of Physician Assistants (NCCPA). Maintenance of required certification is mandatory. 3.2 Minimum of one year of clinical experience. 3.3 Eligibility for privileges and credentialing with Air Force Medical Service, as applicable to field of practice. 3.4 Ability to communicate effectively, both orally and in writing. 4. SPECIFIC TASKS 4.1 Pre Deployment Assessment: Medically evaluate all deploying members on Pre-Deployment requirements to include: 4.1.1 Reviewing medical history, medications, profiles (i.e. AF Form 469 or AF Form 422) to ensure member is medically qualified to deploy. 4.1.2 Coordinate with Primary Care Manager to ensure members are being medically followed up for any disqualifying position pertaining to deployment. 4.1.3 Coordinate with administrator and Public Health Deployment section on notification to the Unit Deployment Manager. 4.1.4 Complete provider review and interview portion of the DD Form 2795 for members endorsing physical and/or mental health concerns, as appropriate to field of practice. 4.1.5 Post Deployment Assessment: Medically evaluated if needed all returning deployed members IAW with current DOD and AF policies. 4.2. Monitor the PDHRA program and coordinate appropriate aspects of the program with the base Installation Deployment Officer (IDO), Unit Deployment Managers (UDMs), Public Health, commanders, Chief of Medical Staff (SGH), and medical clinic personnel. 4.2.1 Maintain knowledge of content and intent of DoD and USAF policy regarding deployment health. 4.2.2 Ensure Installation PDHRA program is administered in accordance with DoD and USAF policy regarding deployment health. 4.2.3 Remain knowledgeable in DoD/Veterans Affairs (VA) Post-Deployment Health Clinical Practice Guideline (PDH-CPG). 4.2.4 Remain updated on PDHRA-process guidance provided by DoD and USAF. 4.2.5 Ensure Military Treatment Facility (MTF) providers conducting PDHRA assessments receive PDHRA clinician training, DoD/VA PDH-CPG training, and other appropriate deployment-healthcare education. 4.3 Remain knowledgeable in the stressors and health threats common to deploying personnel. Maintain proficiency in implementation of interventions or programs that may mitigate such threats or adequately treat deployment-related health conditions. 4.4 Consult with Installation UDMs to ensure redeployed Service members are notified of eligibility for PDHRA in a timely manner and to assist with maintaining satisfactory PDHRA-completion rates. 4.5 Monitor and facilitate processing of DD Forms 2900. 4.5.1 Ensure negative DD Forms 2900 are printed and filed in patients' medical records. 4.5.2 Develop and maintain a system to track processing of DD Forms 2900 and ensure patients are assessed by a provider within the USAF-prescribed timeframe. 4.5.3 Review positive DD Forms 2900. Mental health provider will ensure forms reflecting physical health concerns are immediately brought to the attention of each patient's primary care team for follow-up action. 4.5.4 Contact patients for clinic appointments. Relay general appointment instructions to patients. (Medical appointments will be scheduled for patients with physical health concerns, as well as for patients with both physical and mental health concerns. Mental health appointments will be scheduled for patients with only mental health concerns.) 4.5.5 Regularly document status of open positive DD Forms 2900 via the PDHRA web application in Preventive Health Assessment and Individual Medical Readiness (PIMR) system. 4.6 As appropriate, report PDHRA compliance metrics and program initiatives to installation leadership, such as Population Health Working Group, Community Action Information Board, etc. 4.7 Complete provider review and interview portion of the DD Form 2900 for members endorsing physical and/or mental health concerns, as appropriate to field of practice. 4.7.1 Interview and assess patients regarding post-deployment health concerns. Document interview and assessment on DD Form 2900. 4.7.2 Assess and treat patients in accordance with DoD/VA PDH-CPG. 4.7.3 Ensure patients are provided education regarding their individual deployment-health concerns. 4.7.4 Provide patient referrals for specialty care and ensure follow-up. Document referrals on DD Form 2900 and in patients' electronic medical records. 4.7.5 Sign form to close-out PDHRA record. 4.7.6 Ensure DD Form 2900 is printed and filed in patients' medical records. 4.8 Document clinical encounters in patients' electronic medical records using appropriate codes for PDHRA and deployment-related-condition encounters. 5. The Deployment Health Clinical Center Web site at http://www.pdhealth.mil assists clinicians in the delivery of post-deployment healthcare. 6.
