Description
The Department of Veterans Affairs, George E. Wahlen Medical Center, VA Salt Lake City Healthcare System, 500 Foothill Drive, Salt Lake City, UT 84148, has a requirement for preventive maintenance checks, services and repairs ( full service contract) for ARJO Ceiling Mounted Patient Lifts. The requirement would be for a base year plus 4 option years. All prospective offerors must apply for and receive a Dun and Bradstreet Number @ www.dnb.com, and register at www.ccr.gov. The North American Industry Classification System Code for this solicitation is 423450. Statement of Work The VA requires a full service contract for services, checks and repairs on 42 each ARJO Ceiling Mounted Patient Lifts, models MAXI 600 and MAXI 1000. Prospective vendors will itemize and detail the services and checks that will be provided with the preventive maintenance check and service. Two each preventive maintenance checks and services are required for each contract year. The Preventive Maintenance Check & Service must include as a minimum the following items in accordance with manufacturer specifications and standards so as to protect and ensure compliance with manufacturer warranty and maintenance requirements: Inspect frames, interlocks, hardware, gears for wear, connection joints for proper attachment, trolley and spreader bars. Verify emergency brake and braking devices are working properly Load testing of all lifts Inspect all track and end stops and torque end stops to manufacturer specifications. Inspect and Ensure ceiling brackets and mounting hardware are not displaced and all mountings and attachments meet manufacturer guidelines and health and safety requirements. Inspect all areas required by the manufacturer utilizing the latest manufacturer published updates so that equipment maintains the warrant and ensures patient and hospital staff safety. The requirement is for a base contract of October 1, 2010 to September 30, 2011 with 4 option years. Option year 1 is October 1, 2011 thru September 30, 2012 Option year 2 is October 1, 2012 thru September 30, 2013 Option year 3 is October 1, 2013 thru September 30, 2014 Option year 4 is October 1, 2014 thru September 30, 2015
