- FL-6 FL-100B FL-15 FL-2 FL-1 RFP-090710-RFC - 1,800 PMT's Furnish all labor, materials, services, equipment and facilities necessary to fabricate and deliver: Photomultiplier Tubes (PMTs) or Product No. R7600U-200-M4 with a Head-On type in accordance with the requirements of the attached New PMT Specification (dated 9/01/10) in support of the CMS Operations Program. (Note: Supplier will put "CMS Bar Codes" on the tubes at the factory.) Quote Best Delivery- More specifically, CMS requires a lead time of two (2) months after receipt of order (ARO) at fifty (50) PMTs/month, and then four (4) months ARO at one hundred and fifty (150) PMTs/month. If this lead time cannot be met, please quote the Best Delivery Rate possible that your firm can achieve. Offerors shall respond to each paragraph of the New PMT Specification explicitly acknowledging acceptance of the provisions of that paragraph or taking an exception to the provisions. The reason(s) for the exceptions(s) shall be clearly stated in your Proposal. Note: Attached is a FL-6 Representations, Certifications & Acknowledgments form that must be returned with your Proposal. Quote all prices in U. S. Dollars. If applicable, Duty shall be included in the unit price. Please provide the HTS Code and the applicable Duty percentage. If this PMT is classified as Duty-Free, please state accordingly in your Proposal. Buy America Act: The proposals shall be evaluated as provided in this section so as to give preference to domestic bids. Each bid shall be adjusted for purposes of evaluation by adding to the foreign bid (inclusive of Duty) a factor 6% of that bid, except that a 12% factor shall be used instead of the 6% factor if the firm submitting the low acceptable domestic bid is a small business concern or a labor surplus area concern (as defined in F.A.R. Sections 20.101 and 19.102-2 respectively) or both unless it is determined the award to a domestic concern will result in an unreasonable cost pursuant to FAR 25.105. If the foregoing procedure results in a tie between a foreign bid as evaluated and a domestic bid, award shall be made to the domestic bid.
