- VA-260-10-RQ-1218 Notice Type: Combined Synopsis/Solicitation: This announcement is for a contract to provide lab testing services at the Roseburg VA Medical Center, 913 NW Garden Valley Blvd, Roseburg, Oregon 97471. The contract will be a base and four option years. Contract type will be firm fixed price Non-personal services contract. North American Industry Classification System (NAICS) Codes are 621511 and 622110. Contractor agrees to provide specified laboratory procedures and tests in support of the VA Roseburg Healthcare System Pathology and Laboratory Medicine Services in Roseburg, Oregon. VA Healthcare System Pathology and Laboratory Services 937 NW Garden Valley Blvd. Roseburg, OR 97471 541-440-1000 ext 44724 The anticipated start date of this Contract is January 1, 2011. This Contract will be for an initial period of one (1) year, ending September 30, 2011. Multiple extension options are provided through fiscal year 2015. Contractor shall provide urgent and emergent Laboratory testing services and perform tests as requested by VA Pathology and Laboratory services. Contractor shall provide all labor, equipment, and materials required to perform requested tests. Tests shall be performed to the satisfaction of the VA Roseburg Healthcare System ("VARHS"), Chief of Staff and the Contracting Officer's Technical Representative ("COTR"). Contractor shall provide to the COTR the methodology and ranges for results on any tests that are requested. Professional services provided will be verified by a record keeping system maintained by the Manager of the Laboratory Services at VARHS, which assures that the VA pays for only those services actually requested. The Contractor must be located within the Roseburg, OR city limits to ensure timeliness of lab test results to ensure patient safety.
