Description
DEPARTMENT OF VETERANS AFFAIRS LOUIS STOKES CLEVELAND MEDICAL CENTER VA/VHA VISN 10 Contracting Activity 10000 Brecksville Road Brecksville, OH 44141-3204 JUSTIFICATION AND APPROVAL TO PROCURE USING OTHER THAN FULL AND OPEN COMPETITION Upon the basis of the following justification, the use of other than full and open competition for the proposed contractual action is hereby approved pursuant to the authority of 41 U.S.C. 253(c)(1) and FAR 6.302-1, One Responsible Source and no other supplies or services will satisfy agency requirement. JUSTIFICATION 1.Identification of Agency and Contracting Activity. The Louis Stokes Cleveland Department of Veterans Affairs Medical Center, 10000 Brecksville Road, Brecksville, OH 44141-3204, VISN 10 contracting Activity. 2.Nature/Description of Action. This activity proposes to fund Pathology service with an undefinitized purchase order with the Blood Center of Wisconsin for one year of third party diagnostic laboratory testing under decentralized authority to the Pathology and Laboratory Management Service. The period of performance is October 1, 2010 to September 30, 2011. 3.Description of Supplies/Services. This requirement is to allow the Cleveland VAMC's Pathology & Laboratory Management Services to maintain the level of patient care services required by the Medical Center's Quality Assurance Management. Blood Center of Wisconsin Laboratory Service will provide such critical services as esoteric laboratory testing and pathology consultation and services. The estimated cost is not to exceed $50,000.00. 4.Identification of Statutory Authority. The statutory authority permitting One Responsible Source procurement is 41 U.S.C. 253(c)(1) and FAR 6.302-1. 5.Rationale in support of citation in Paragraph 4 above: Description of contractor's unique qualifications or the nature of the acquisition requires use of the authority cited: Blood Center of Wisconsin has been the provider of these services under a monthly procurement. This requirement will for fill the agency temporary need. This request covers the period of performance from October 1, 2010 to September 30, 2011, while the pending solicitation for esoteric laboratory service is ongoing. It is anticipated that this need will terminate upon the awarding of a VISN wide esoteric laboratory BPA. Failure to procure Pathology and Laboratory testing services for the hospital would severely impact patient care. 6. Federal Business Opportunities (Fedbizops) Announcement/Potential Sources. This requirement will not be synopsized in accordance with FAR 5.202(13)(i), the proposed contract action is for an amount not expected to exceed the simplified acquisition threshold. 7. Determination of a Fair and Reasonable Cost The Contracting Officer has determined that the anticipated cost to the Government will be fair and reasonable based on price analysis of previous purchases. 541-C00625 $19,000.00. 8. Description of market survey. This requirement will not be synopsized in accordance with FAR 5.202(a)(13)(i). 9. Additional Support for the use of other than full and open competition: None. 10.
