Description
Revised Statement of Work dated 21 October 2010 Statement of Work for the Contractor 1.0. DESCRIPTION OF SERVICES 1.1. Scope of Work. Contractor shall provide telephone support/counseling for the smoking cessation program. Contractor shall provide a live operator and a toll-free tobacco cessation quit-line. 1.2. Basic Services. The Contractor shall provide telephone support/counseling for the smoking cessation program. 1.2.1. Contractor shall provide access to registered nurses and/or respiratory therapists as counselors via a toll-free tobacco cessation quit-line. Each caller shall receive individualized tobacco cessation counseling and a free packet of information outlining the cessation process. The Contractor specialists will inform callers of the tobacco cessation medications available to them and explain medication therapy. 1.2.1.1 At the end of each phone conversation, it is the responsibility of the contractor to email the HAWC at the designated email addresses, a copy of the Subjective Objective Action Plan(SOAP) note detailing the call and needs of the client. 1.2.1.2 Although most clients will be able to quit within a 3-4 month period, the contractor does not limit the time involved with a client. 1.2.1.3 Fax referral form, provided by the contractor may be used by the HAWC to refer callers to the telephonic counseling services. 1.2.1.4 The contractor will provide the HAWC with a quarterly list of the clients who have participated in smoking cessation counseling. 1.2.2. If the patient requests tobacco medications or replacement therapies (Chantix, Zyban or nicotine patch), they are required to attend the Langley Tobacco Cessation course and call the Contractor weekly. They are welcomed to call more frequently. 1.2.3. The Contractor will provide a detailed listing of members remaining current and active in the program via an established email account for local follow-up. 1.2.4. The Contractor will provide data on three and six month quit rates of participants in the Tobacco Cessation program. 1.3. Hours of Operation. The Contractors shall provide services Monday - Friday between the hours of 7:00 A.M. and 9:00 P.M. CST on the phone number 1-800(no cost) access to clients once enrolled in the smoking cessation program by Langley AFB. 2.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES 2.1. Government Furnished Property. N/A 3.0. GENERAL INFORMATION 3.1. Account Manager. Contractor shall provide an Account Manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The name of this person and an alternate, who shall act for the contractor when the manager is absent, shall be designated in writing to the Contracting Officer (CO) within five days of contract award. The account manager and alternate must be able to read, write, speak and understand English. 3.1.1. Availability of Account Manager. The Account Manager or alternate shall be available 7:30 AM through 4:30 PM Monday through Friday, for contact by telephone, to resolve any problems that may arise. 3.2. Working Hours. The Contractors shall provide services Monday - Friday between the hours of 7:00 A.M. and 9:00 P.M. CST. 3.3. The following Federal Holidays are observed at Langley AFB, VA. The contractor is not required to perform services on these Federal Holidays.
