- Standard Form 1449 Solicitation body (SOW, Terms and Condiitons) Invitation Letter This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the Federal Acquisition Regulations (FAR) Part 12 and Part 13, as supplemented with additional information included in this notice. The Social Security Administration (SSA) in Region 8 intends to award multiple Blanket Purchase Agreements (BPA) with licensed physicians to provide medical consultant services in the following specialties: Family Practice Medicine; Internal Medicine; Occupational Medicine; Physical Medicine/Rehabilitation; Psychiatry; and Psychology. SSA's Center for Disability utilizes the services of physicians in review of disability claims. Services will be performed on site at SSA Region 8 Denver office. These consultant services will include providing oral and/or written consultations resulting from reviews of SSA disability case files, consulting with SSA medical policy staff and/or training other medical consultant contractors. No patient contact will be involved. New medical consultants (those not currently under contract to provide these services to the SSA) will be required to participate in an orientation program which is intended to enable the contractor to attain and achieve SSA's performance expectations. The BPA's period of performance is January 1, 2010 or date of award, whichever is later, through December 31, 2015. The Solicitation Number for this requirement is SSA-RFQ-R08-11-1002. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-46. The associated North American Industrial Classification Code for this requirement is Office of Physicians (except Mental Health Specialist) is 621111 and Office of Physicians (Mental Health Specialist) is 621112, with a small business standard of $10 million. This requirement is a 100% Small Business Set Aside. Offers received from other than small business concerns will be deemed unacceptable and will be given no further consideration. The selected offeror must comply with the following commercial item terms and conditions as well as any addenda to these clauses. The FAR clauses and provisions that apply to this acquisition are FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items. In addition, offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with its proposal. Applicable terms and conditions, and representations and certifications are attached. The Government will award BPAs resulting from this solicitation to the responsible offeror whose offer conforms to the requirements of this solicitation To be eligible for an award, Offerors must be registered in the Central Contractor Registration (CCR) database. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov. All awardees will be required to undergo a security background screening which will include completion of fingerprint cards before beginning performance under the contract Quotations in response to this combined synopsis/solicitation are due no later than 5:00pm EST on December 6, 2010. Offerors may send responses via email to Lisa.Walker@ssa.gov. All questions regarding this RFQ must be submitted in writing by November 17, 2010 at the email address provided previously. NO PHONE CALLS WILL BE ACCEPTED. E-mail communications shall include the offeror's name, solicitation number, e-mail address, phone and fax numbers. The Government will make every attempt to answer questions which will not automatically result in the extension of the proposal due date. All questions and their respective answers will be made available to all prospective offerors in the form of solicitation amendments. Also, a pre-proposal conference is scheduled for November 19, 2010 at 1:30pm, at SSA Denver Office you must RSVP for the meeting by email no later than November 16, 2010 to Lisa.Walker@ssa.gov
