- Provide Infectious Disease Services to include diagnosis and treatment of infectious disease conditions of referred patients from VA Manchester. The Physician will be required to provide on site and telephone consultations for patients requiring ID services as referred by their VA Provider. As part of the care provided, the contract physician is requested to prescribe drugs within the formulary list of the Manchester VA Facility whenever clinically appropriate, when needed a non- formulary request will be submitted by the requesting provider. This contract will have a base of one year with options to renew for four (4) additional one year periods. The solicitation package will be available on or about November 19, 2010. The acquisition will be issued as unrestricted. The NAICS Code for this procurement is 622110. Proposals are due on December 10, 2010 by COB. Award will be made on the best value to the Government. Evaluation factors will include education, past performance, and price. A Firm, Fixed-Price contract award is anticipated.
