- Fuji x-FCR Go Digital Portable X-ray system or equal. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation (Request for Quotation) reference number is FA5205-11-Q-RK01. The solicitation incorporates provisions and clauses as those in effect through Federal Acquisition Circular 05-38. This procurement is NOT a set-aside. The North American Industry Classification System code (NAICS) is 423450 - Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers. Offerors are required to forward proposal and technical documentation by 1630 JST December 01 2010. DESCRIPTION: Fuji x-FCR Go Digital Portable X-Ray System or equal for Misawa AB Japan. Please refer to the attached RFQ No FA5205-11-Q-RK01. PROVISIONS/CLAUSES: The following FAR and DFARS provisions and clauses apply: 252.203-7000, 252.232-7003, 52.204-6, 52.204-7, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.225-13, 52.232-33, 52.212-5, http://farsite.hill.af.mil. ADDITIONAL NOTES: Include your DUNS number, CAGE code and Federal TIN in your quote. To obtain or renew a DUNS number or CAGE code, visit www.ccr.gov. Ensure your company's Representations and Certifications are current and up to date for this acquisition.
