Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format of FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation will not be issued for the solicitation number NICHD-11-030-1. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC2005-46. This acquisition will be processed under Simplified Acquisition Procedures and issued as a Request for Quote (RFQ). The associated North American Industry Classification System (NAICS) Code is 811219 which has a size standard of $19.0M dollars. This solicitation is not a set aside for small business. The Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), National Institutes of Health (NIH), plans to procure Maintenance Service on a SP5 + UV/405 + 405 Laser + AOTF + WLL Microscope. Period of Performance will be for a minimum of one (1) year up to a maximum of three (3) years. The vendor shall be capable of doing the actual repairs and providing all the needed parts for repairs, phone, software support and upgrades for the Microscope as well as the lasers. The maintenance shall provide coverage to include the following: 1) On-site labor, travel time, and travel expenses 2) On-call service provided from 8:00AM to 5:00PM during normal workdays, Monday through Friday, excluding observed holidays 3) Parts replacement as required on an exchange or new part basis, excluding consumables and objectives 4) One preventative maintenance inspection per annual maintenance agreement 5) Trained Service Representatives to perform axial resolution measurements, as well as laser power measurements to confirm optimum system performance 6) Toll free, expert engineering and applications support provided through a technology support center 7) UV Ready Coverage to include couplings, fibers, pinhole optics, UV Lens wheel/Filter wheel, UV AOTF, and labor/travel expenses associated with UV alignment and repair The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-1 Instructions to Offerors - Commercial Items; FAR Clause 52.212-2, Evaluation - Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions applies to this acquisition. The offeror must include their Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Interested vendors capable of furnishing the government with the service specified in this synopsis should submit two (2) copies of their quotation to the below address. Offerors quotation shall include the cost for each year separately up to the three year maximum. Offers must also be accompanied by descriptive literature, warranties and/or other information that demonstrates that the offer meets all of the foregoing requirements. Quotations will be due fourteen (14) calendar days from the publication date of this synopsis or by December 23, 2010 by noon (12:00p.m.) EST. The quotation must reference "Solicitation number" NICHD-11-030-1. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations must be submitted in writing to the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) 6100 Executive Blvd, Room 5Z00, Rockville, MD 20852 Attention: Tracey Muck.
