- SF 33 Incumbent Information Dept of Labor Wage Determination Inventory Training Subcontracting Form Lobbying Form Past Performance Solicitation Document Ladies and Gentlemen: You are invited to submit a proposal in accordance with the requirements of Request for Proposal (RFP) 11-233-SOL-00020, NATIONAL MEDICAL RESPONSE TEAM- NATIONAL CAPITAL REGION. To respond to this RFP, you must submit separate technical, past performance and price proposals. Sections L and M of the RFP contain information, instructions and the evaluation criteria that will be used to evaluate your proposal. Your proposal must be received by the Contracting Office no later than 3:00 p.m. E.S.T., January 20th, 2011, at: Department of Health and Human Services, PSC Division of Acquisition Management, SAS Parklawn Building, Room 5C-18 5600 Fishers Lane Rockville, Maryland 20857 Reference: (RFP) 11-233-SOL-00020, NATIONAL MEDICAL RESPONSE TEAM-NATIONAL CAPITAL REGION. Because of the increased security at Federal buildings, please allow extra time for your proposal to be delivered to the Contracting Office. The Division of Acquisition Management will no longer guarantee that personnel will be available to accept hand carried/courier packages. Every reasonable attempt will be made to accommodate all deliveries but the government maintains that it is the Offeror's responsibility to get all proposals to room 5C-18 by the scheduled closing date and time. Proposals received late in the Contracting Office due to a delay caused by these security measures are considered late under FAR Clause 52.215-1 "Instructions to Offerors-Competitive Acquisitions (JAN 2004)(c)".
