- Appendix A- QASP Request for Proposals Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.3, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (ii) The solicitation number 2011-N-13059 is being issued as a request for proposal. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. (iv) This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 322211 with a size standard of 500 employees (v) Information concerning the contract line item numbers and items, description of services, dates and places of delivery are included in the attached request for prosposal. (vi) Description of requirements and places of performance can be found on the attached request for quotation, Section C -Requirements Statement.. (vii) The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. (viii) The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The evaluation procedure to be used for this procurement is price and other factors. The evaluation factors can be found in the statement of work. (ix) The Government will award a firm-fixed price indefinite delivery indefinite quantity contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award shall be based on Best Value to the Government. (xi) Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xii) Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.219-6, 52.219-28, 52.222-3, 19, 21, 26, 35, 36, 37, 50, and 52.232-33. (xiii) Clauses 52.252-2, 52.204-7, 52.204-9, 52.216-18, 52.216-19, 52.216-21, 52-217-8, 52.217-9, 52.223-5, 52.225-13, 52.228-5, 52.323-8, 52.247-34, 52.253-1, apply to this acquisition. (xiv) Clause 52.222-41, Service Contract Act of 1965. (xv) Questions must be received by 2:00 pm Eastern Standard Time on December 28, 2010. (xvi) All offerors must submit a proposal by 2:00 pm Eastern Standard Time January 07, 2011. Proposals received after that time may not be considered.
