- This is a Sole Source combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-46. The associated North American Industrial Classification System (NAICS) code for this procurement is 518210 with a small business size standard of $25 million. The period of performance shall be 02/01/2011-09/30/2011. The Centers for Disease Control and Prevention, Atlanta, GA, intends to procure on a sole source basis Medical Record Chart Abstraction from Inpatient Charts and Out-Patient Charts to be procured from Buccaneer Computer Systems. Buccaneer Computer Systems is the only company able to provide this service due to the fact that this is a continuation of an existing project previously funded through an IAA to Centers for Medicare/Medicaid Services (CMS), who then contracted to Buccaneer for the work to be performed. The current SOW is a continuation of a previous project (e.g., Assessment of CABG) involving the assessment of infections using claims data. As a result, previous experience with record requests for inpatient and outpatient facility procedures identified under the Surgical Care Improvement Project (SCIP) is needed, and vendor must have an individual available with expertise in reviewing and abstracting information from charts when clarification is needed. Buccaneer is the only vendor that has access to the required charts needed for abstraction and has an individual trained to review the hard copy charts, as they have completed Phase 1 of this existing project. This project is supported by justification for other than full and open competition to Buccaneer under authority of FAR 13.106-1(b)(1). Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 5 days from the posting of this notice, their written capabilities statement and pricing information. Please forward the capability statement and pricing information to Maria Shamburger, Procurement and Grants Office, 2920 Brandywine Rd, Atlanta, GA 30341, Reference 2011-93544. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all capabilities statements and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement. The following provisions apply to this commercial solicitation: 1. FAR clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and a statement regarding any addenda to the provision. 2. FAR clause 52.212-2, Evaluation -- Commercial Items, if used, and the specific evaluation criteria to be included in paragraph (a) of that provision. If this provision is not used, describe the evaluation procedures to be used. 3. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 4. FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. 5. FAR clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. 6. Additional contract requirement(s) or terms and conditions may be required for this acquisition if determined necessary by the contracting officer and consistent with customary commercial practices. If no affirmative responses are received within 5 days, in accordance with FAR 13.406-1(b)(1), negotiations will be conducted with Buccaneer as the only source and a purchase order will be issued without any additional notices being posted.
