- Amendment 0002 RFQ HT9402-11-Q-0003 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; quotations are being requested and a written solicitation will be issued as request for quotation (RFQ) Number HT9402-11-Q-0003. This is a requirement for an Electric Carousel Lateral File system designed for the storage of paper files. See the attached RFQ HT9402-11-Q-0003 for the contract line item description, clauses and provisions, evaluation criteria, and additional information. Delivery and acceptance is FOB Destination, Within Consignee's Premises, to the following location: TRICARE Management Activity, 16401 East Centretech Parkway, Room 222, Aurora CO 80016-9066. The solicitation/request for quotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-47, effective on December 13, 2010. This requirement is a small business set-aside based on the associated NAICS Code 333922, Conveyor and Conveying Equipment Manufacturing, with a business size standard of 500 employees. The provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. This award will be a firm-fixed price contract based on the criteria specified in RFQ HT9402-11-Q-0003. Offerors must provide a response to each specification, and if no response is provided, the offeror's quote will not be evaluated nor considered for award. Offerors are advised to include a completed copy of the provision 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. The clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items, and the additional FAR clauses cited in the clause are applicable to the acquisition. The contracting officer has determined additional FAR and DFARS clauses and contract requirement to be necessary for this acquisition and consistent with customary commercial practices. There is a site visit for interested parties scheduled for Wednesday, January 12, 2011 at 11:00 AM Mountain Time. Potential offerors are encouraged, but not required to attend the site visit. The site visit will provide an opportunity to take measurements and to view the delivery and installation areas. All questions, must be submitted by email only to: kathleen.hamilton.ctr@tma.osd.mil and scott.lamond@tma.osd.mil, by the cut off date of January 21, 2011 at 1:00 PM Mountain Time. Questions received after the cut-off date may not be addressed. QUESTIONS RECEIVED BY PHONE WILL NOT BE ADDRESSED. Responses to questions will be posted on the Federal Business Opportunities website as an amendment to this RFQ Signed and dated quotations shall be received by 1:00 PM Mountain Time, Friday, January 28, 2011. Interested parties must provide the name(s) of those attending by email to Ms. Kathleen Hamilton at kathleen.hamilton.ctr@tma.osd.mil and Mr. Scott Lamond at scott.lamond@tma.osd.mil by 2:00 PM MT, Tuesday, January 18, 2011. All visitors are required to show valid state or federal issued photo identification in order to access the building. SUBMISSION OF QUESTIONS All questions, including those resulting from the site visit, must be submitted by email to Ms. Kathleen Hamilton at kathleen.hamilton.ctr@tma.osd.mil and Mr. Scott Lamond at scott.lamond@tma.osd.mil by the cut off time of 1:00 PM MT on January 21, 2011. Questions received after this time may not be addressed. Offerors must acknowledge receipt of this amendment as specified in Block 11 of the attached Standard Form 30. Amendment 0002 is issued to respond to questions submitted regarding Solicitation HT9402-11-Q-0003. Offeror must acknowledge receipt of all amendments as specified in Block 11 of the Standard Form 30.
