- Form 2345 Instructions for Form 2345 REQUEST FOR INFORMATION (RFI) Concerning the acquisition of a Dynamic Brake Test Stand for the 309th Commodities Maintenance Group of the 309MXW at Hill AFB as described in the attached purchase specification. Interested vendors/contractors need to submit a current signed and stamped DD Form 2345 in order to receive the T.O.'s referenced in the purchase specification. Please see the attached instructions and Form 2345. All interested parties are requested to send answers to the questions below via email to Tamara.Yockey@hill.af.mil by 5 p.m. MST, February 4, 2011. INDUSTRY DAY An Industry Day will be held in February to discuss the objectives/requirements of the program and to answer questions arising from this RFI. As dates, times, and details are confirmed they will be posted to Fedbizopps. Check the web site often for updates. RFI QUESTIONS Answers to the following questions facilitate discussion and influence the RFP. They may aid in developing evaluation criteria for the upcoming competition. To ensure that your answers have the opportunity to influence this acquisition, please submit your responses by 5:00 pm MST on February 4, 2011. In addition to your responses to the RFI questions, feel free to include any suggestions, recommendations, questions, or concerns regarding this effort. The team intends to address identified concerns/suggestions at industry day during the Q&A session. The information provided is for the Air Force's market research and is given understanding the AF has no obligation to pay for the information. The information may be used in developing a request for proposal (RFP) but does not obligate the AF to issue an RFP or contract. Any proprietary information submitted may be used as a general concept but specific information will be safeguarded as proprietary, if so marked. Requested Information: 1. What is your company name and CAGE code? 2. Who is your point of contact? List email address, telephone number and any other pertinent contact information. 3. What NAICS code do you suggest for this acquisition? 4. Is your company a small business for this NAICS Code? If your company is a small business, is it also a woman-owned, small disadvantaged, 8a, HUB Zone, or service disabled veteran owned business? 5. If a small business, would your company provide products of its own manufacture or would it provided products manufactured by another small business? 6. Is your interest in this as a prime contractor or as a subcontractor? 7. Describe financing methods customarily provided by buyers in this industry (industry defined by the NAICS code or by providing this type of requirement). Note: the Air Force (AF) is not authorized to make advance payments (payments before costs are incurred or work is done). 8. Do you consider the requirement to be commercial (see FAR Part 2 definitions)? 9. Are there any commercial items that exist which meet the purchase specification? 10. Is there a commercial product that with modification can meet the purchase specification? 11. How much time is needed to create a proposal based on the provided purchase specificiation? 12. What is your estimated cost for this Dynamic Brake Test Stand as detailed in the purchase specification? 13. How long do you think it will take to build this item? How much time will it take to install it? 14. Are you willing to grant government purpose rights for any data required for this acquisition? 15. Have you ever been awarded a Government contract? If so, with what Agency? 16. Describe your experience of same or similar requirements. Include quantities, customer, and date of delivery and period of performance. 17. Discuss any concerns you may have with our requirement or specification.
