Description

Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial services prepared in accordance with the guidelines and format set forth in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, VA118-11-RQ-0153, is being issued as a request for quotation (RFQ) from the Department of Veterans Affairs (VA), Office of Acquisition and Logistics, Technology Acquisition Center. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2009 and the latest version of the VA Acquisition Regulation (VAAR) supplement, current as of December 8, 2009. This RFQ is issued on a non-competitive, sole source basis to Clinical Evaluations, LLC, under the authority of 38 U.S.C 8127 (c), Veterans First Contracting Program. Period of Performance: January 31, 2011 - May 31, 2011 Inspection and Acceptance: Destination FOB: Destination The contractor is required to complete and submit a copy of the FAR Provision 52.212-3, Offeror Representations and Certifications - Commercial Items, if not already on file at the Online Representations and Certifications Applications web site http://orca.bpn.gov All offers must be submitted to Elizabeth.dean2@va.gov no later than 12:00 PM EST on 28 Jan 11. All offers must provide a breakdown of all costs to include but not limited to proposed labor categories, labor hours, labor rates, other direct costs, indirect rates and profit.
