- NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention, National Institute for Occupational Safety and Health (NIOSH), Office of Administrative and Management Services (ASB), hereby announces its intent to issue a purchase order to Flexible Lifeline Systems for rooftop fall protection systems repairs. Purpose and objective: The Fall Protection Systems allows the user to walk uninterrupted the entire length of the system and provide secure anchorage to arrest a fall by the users. Repairs are needed to existing Latchways Fall Protection Systems that were designed and installed by Flexible Lifeline Systems on buildings 29, 148, 152, 153, 154, 155 & 166. These systems were damaged due to heavy snow and ice build up after a major winter storm which occurred in February of 2010. Flexible Lifeline Systems is certified to work on Latchways® Systems. Flexible Lifeline Systems designed, installed and certified (meet all local and federal safety regulations) for each rooftop protection system. Includes the following: B. 29 - Lifeline System 3344-3; Design four (4) replacement support brackets in order to repair this system. B. 148 - Lifeline Systems 10291-3 & 10291-4; Design two (2) replacement support brackets for 10291-3 plus two (2) replacement support brackets for 10291-4 for a total of four (4) new support brackets in order to repair these systems B. 152 - Lifeline Systems 3344-5 & 3344-6; Design six (6) replacement support brackets for 3344-5 plus two (2) replacement support brackets for 3344-6 for a total of eight (8) new support brackets in order to repair these systems. B. 153 - Lifeline System 10291-5; Design three (3) replacement support brackets in order to repair this system. B. 154 - Lifeline System 10291-6; Design four (4) replacement support brackets in order to repair this system. B. 155 - Lifeline Systems 3344-10 & 3344-11; Design two (2) replacement support brackets for 3344-10 plus two (2) replacement support brackets for 3344-11 for a total of four (4) new support brackets in order to repair these systems. B. 166 - Lifeline System 3344-7; Design three (3) replacement support brackets in order to repair this system. Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written item description/item characteristics and pricing information in the format they choose, not to exceed 10 pages. Please forward the item description and pricing information to Margaret L. Mooney, CDC NIOSH, 626 Cochrans Mill Road, Pittsburgh, PA 15236, Reference 2011-Q-13244. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all item descriptions and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with Flexible Lifeline Systems, 14325 West Hardy Road, Houston, TX 77060 as the only source and a purchase order will be issued without any additional notices being posted.
