- PLEASE NOTE THIS IS A REQUEST FOR INFORMATION ONLY. NOT A REQUEST OF PROPOSAL. Contractor having the capabilities necessary to perform the stated requirements should send an email ONLY to gary.rogers4@va.gov with the following information: 1. Certified business type (i.e. small, large, small disadvantaged, woman owned, veteran owned, 8(a) minority owned). Please respond no later than 3/16/11. It is contemplated that a requirements contract will be awarded from this solicitation to provide for hotel rooms and specified meals for the Department of Veterans Affairs National Veteran TEE Tournament. The performance period of the contemplated contract is September 12, 2011 through September 15, 2011, with deliveries or performance to be scheduled by placing orders with the contractor in accordance with the scope of work. Orders may be placed by the Contracting Officer after contract award, but not prior to August 16th. The following is the estimated total quantity of rooms and specific meals identified in this schedule. These estimates are not a representation to the contractor that the estimated quantities will be required or ordered, or that conditions affecting requirements will be stable or normal, but are reflective of historical data and current estimated needs. STATEMENT OF WORK The National Veteran TEE (Training, Exposure, Experience) Tournament, (NTVTEE), is a golf rehabilitation program that includes other recreational activities such as: bowling, kayaking, horse shoes, bag toss, for Veterans who are legally blind, amputees, those that use wheel chairs, and other veterans having various other disabilities. The NVTEE Tournament uses a therapeutic format to promote rehabilitation, fellowship, and camaraderie among its participation. 1. ROOM/EVENT REQUIREMENTS: a.The Department of Veterans Affairs requires the use of furnished hotel or motel rooms for the National TEE Tournament Event held the second full week of September 12-15, 2011. The rooms offered shall be within a fifteen (15) miles driving distance of the VA Medical Center, Hwy 6 West, Iowa City, Iowa 52246 for September 12-15, 2011. b.The minimum requirement of 20 shower chairs needs to be made available for use as needed to VA referred TEE Tournament occupants. c.The fully furnished hotel rooms will include contractor provided utilities and other specified services. The hotel rooms shall comply with all city, county and state building codes and ordinances applicable to the ownership and operation of the building in which the rooms are located. Rooms shall comply with the applicable NFPA 101, 2000, Chapter 29. Must have a minimum of 8 handicap accessible rooms. d.Contractor shall provide on-site facilities to accommodate up to 425 veterans and guests for meals and other related event activities such as golf, golf instruction, kayaking, bowling, bag toss, casino night, and other entertainment events. e.Contractor shall provide dinner for 350 on Monday, (9/12), dinner for 350 on Tuesday, (9/13) and 425 on Thursday (9/15). Breakfast buffet shall be provided Tuesday through Friday, (9/13 thru 9/16). All dinners shall be seated, hot buffet style meals and held in the "Robert Buffet" area of the hotel. f.Contractor shall have the "Convention" area available for a Casino Night Program, (9/14/11, 7-9 PM) provided by the NVTEE for participants and accompanying companions. A small stage area shall be available for scheduled entertainment. g. The Contractor shall supply additional support rooms for the event: (1) one storage area (min. size 10x12), (1) one medical room (min. size 10x12), (1) one media room (min. size 20x20), (1) one training room (min. size 10x12), (1) one command center (min. size 10x12). Rooms must be capable of being locked and secured. h.The Contractor shall provide a minimum requirement of 150 parking spaces for the period of the contract. i.The Contractor shall provide space available for stage show on Monday and Thursday night of event (stage handicap accessible) and for up to 425 seating capacity. j.The Contractor shall provide handicap access to load and unload buses for event. k.The Contractor shall provide a Lake area and services available for Kayaking sessions/instruction. l.The Contractor shall provide a Golf Course and instructional practice area. m.The Contractor shall provide a Golf Swing Studio and specialized equipment/personnel to assist in instruction. n.Contractor shall supply all utilities including heat, air conditioning, electricity, water and sewer, local telephone service, and basic television cable service. o.Dinner meal on 9/12/2011 should be seated buffet style hot meal. Suggested menu; Beef Medallions, Potatoes, Fresh Vegetables, Rolls, Salad, Desert, Drinks in accordance with all terms and conditions contained herein. p.Dinner meal on 9/13/2011 should be seated buffet style hot meal. Suggested menu; Pork Loin, Salad, Potatoes, Fresh Vegetables, rolls, Desert, Drinks in accordance with all terms and conditions contained herein. q.Dinner meal on 9/15/2011 should be seated buffet style hot meal. Suggested menu; Fillet of Beef Tenderloin, Potatoes, Vegetables, Rolls, Desert, Drinks in accordance with all terms and conditions contained herein. r.Sponsor meal event will be held on 9/12/2011 at 12:00 pm. This event is to recognize support from the National Sponsor Package of the NVTEE Event. Suggested menu; Sliced Flank Steak, Potatoes, Fresh Vegetables, Rolls, Salad, Drinks in accordance with all terms and conditions contained herein. s.Breakfast meals will be served at Hotel on 9/13 thru 9/15/2011. Breakfast menu would include items on morning buffet in accordance with all terms and conditions contained herein. t.Lunch meals will be served at Hotel on 9/13 thru 9/15. Suggested menu: Cold Cut sandwiches, chips, fruit or cookie and drinks in accordance with all terms and conditions contained herein. 2. ROOM RESERVATIONS a.Room reservations, room occupancy, and room payment will be on the following basis: (1)The Contractor shall allow the occupancy of the room covered by this contract only upon the specific individual referral by the Contracting Officer's Technical Representative (COTR). The COTR will inform the Contractor of each individual's name and the expected arrival time at the Contractor's motel/hotel. (2)For this contract, the Contractor shall use its existing check in and check out times. b.Either the COTR and the TEE Tournament Assistant or VA referred occupant will notify the Contractor of when specific individuals will be checking out of the hotel/motel. In the event that a VA referred occupant refuses to leave the room, the Contractor shall immediately notify the COTR. Also, in the event that a VA referred occupant checks out of the hotel/motel without prior notice from the VA or the occupant the Contractor shall immediately notify the COTR or the TEE Tournament Assistant upon early departures. c.Gary Rogers is the official Government Contracting Officers representing the Iowa City VA Medical Center authorized to place orders under this contract. 3. PATIENT EMERGENCIES: In the event a VA referred occupant is in an emergency situation of any kind, the Contractor shall call 911to respond to the emergency. In addition, the Contractor shall immediately then contact the VA Medical Center COTR with applicable contact numbers or the VA Medical Center, on site Medical Team, Officer of the Day if the emergency occurs in other than normal business hours. 4. There shall be no additional charges to the room occupants for lodging. Any expenses incurred in addition to those covered by the lodging (e.g. unauthorized meals, damages, movie channels, room services, long distance calls, etc.) are not the responsibility of the Iowa City Healthcare System or the NVTEE Tournament but that of the occupant. Local calls are included in the lodging rate. 6.Fire Safety (1)Buildings in which space is offered shall be complaint in accordance with latest editions of the National Fire Codes and Uniform Building Code. (2)Equipment, services, or utilities furnished and activities of other occupants shall be free of safety, health, and fire hazards. When hazards are detected, the contractor shall promptly correct hazards. (3)Maintenance of contractor-owned fire extinguishers shall be provided by the contractor in accordance with NFPA Standard No. 101. (4)As required by code, smoke detectors/alarms shall be installed in each room, office, storage area, hallways and other common areas. 7.Asbestos - No asbestos containing fireproofing or insulation on building structures, acoustical treatment, molded or wet-applied ceiling or wall finishes/decorations, will be permitted. If present, such materials shall be removed by the contractor prior to occupancy by the VA. 8.Wheelchair Accessibility: All areas and furnishings must comply with Uniform Federal Accessibility Standards (UFAS). 9.Elevators: If elevators are a part of the room complex, they shall conform to the latest requirements of the American National Standard A171 and A171a Safety Code for Elevators, except for elevator emergency recall and shall be inspected and maintained in accordance with American National Standard A17.1, Inspector's Manual for Elevators or equivalent local code. The elevators shall also comply with local codes and ordinances. 10.Miscellaneous: No activity to the building and/or grounds that would cause an increase in dust, dirt or spores to be airborne (i.e., renovation to the building, movement of earth on the grounds) shall take place without the contractor's written notification to the VA at least seven (7) calendar days in advance of the event.
