Description
1) Type of Notification: Sources Sought (FBO) This is a sources sought synopsis only, (not a pre-solicitation notice pursuant to FAR Part 5). The purpose of this notice is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a) company name or individual physician name (b) address, (c) point of contact, (d) "number of employees" as defined by the Federal Acquisition Regulation (FAR) part 19.101 and (e) the amount of "annual receipts" as defined by the FAR part 19. In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor the work described below. 2) Description: Personal Services (Neuro-Ophthalmologist) The following is a brief summary of the primary duties that the Neuro-Ophthalmologist must perform: Evaluating all patient refered with Neurophthalmologic diseases and disorders. This will include one day per week of Neurophthalmology clinic (including performance of Neurophthalmologic, pediatric and strabismus, and general ophthalmologic surgical procedures as needed). The Health Care Provider shall provide direct resident education during the Neurophthalmology teaching month based upon the resident education schedule. This statement is a projected need for Wilford Hall Medical Center and is subject to change in future postings for this requirement. 3) Hours of Performance: To Be Determined, Please take notice that this may not be a full time position. At this time we are waiting for confirmation that this requitement will be supported. 4) Qualifications: Neuro-Ophthalmologist is a physician who has completed a residency either in neurology or ophthalmology, followed by 1-2 years of fellowship training in Neuro-Ophthalmology. They deal with diseases of the optic nerves, cranial nerves 3,4,5 and 6; pupillary abnormalities, eye movement abnormalities and nystagmus, disorders of visual processing centers in the brain and visual problems in patients with certain neurological diseases. The assessments are usually longer than a typical ophthalmological exam as they do both an eye exam AND a neurological exam; so they see far fewer patients than a general ophthalmologist. The typical Neuro-Ophthalmologist case is a patient who has already seen 1-2 Ophthalmologist and neurologists, with no diagnosis. 5) Place of Performance: These services would be performed at Wilford Hall Medical Center and/or military installations in San Antonio, TX. This survey is to determine the capability of potential sources and the availability of commercial sources and services pursuant to FAR Part 10. This notice is for informational purposes only and does not constitute an Invitation for Bid nor Request for Proposal, and is not to be construed as a commitment by the Government. The Government will not reimburse contractors for any cost incurred for their participation in this survey. Large and small businesses are encouraged to participate in this survey. In the event a prospective vendor is selected for an award, the selected vendor must be registered in the Central Contractor Registration at www.ccr.gov.
