- Schedule of Items Diobsud Creek Concrete Box Culvert and Jersey Barriers Offerors Representations and Certifications The Mt. Baker-Snoqualmie National Forest intends to award a contract for manufacture and delivery of a 12 ft span X 6 ft rise, 25 ft long concrete box culvert, (2) standard 12 ft long jersey barriers, (4) jersey barrier terminal sections and anchoring hardware. Upon delivery of the all requirements and acceptance, payment for the purchase will be via by Government credit card or by Purchase Order. As a Federal Agency, we do not pay State or Local sales tax. MATERIAL PROPERTIES & REQUIREMENTS: See Attachments 03 DELIVERY LOCATION, DATE INFORMATION: See Attachment 03 CERTIFICATION: A signed certification shall be required upon delivery. SHOP DRAWINGS: Contractor shall furnish shop drawings along with assembly instructions. TECHNICAL CONTACT: The technical contact for this project is Jim Mitchell of the Mt. INTERESTED VENDORS: Offerors are to provide pricing for Items as outlined in Attachment 01 Schedule of Items along with a completed copy of Attachment 02 the provision 52.212-3 Offerors Representation and Certifications- Commercial Items with its offer RESPONSE DATE: All quotations received by April 22, 2011 by 3:00 pm PST will be considered by the Government. Responses may be faxed to (425) 783-0129 or emailed to jhada@fs.fed.us. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation number is AG-05M6-S-11-0018 and is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51. The North American Classification System code is 327332 with a corresponding size standard of 500 employees. This project is a 100% set aside for small business concerns.
