- BPA Terms and Conditions Performance Work Statement describing what will be required through the duration of this Blanket Purchase Agreement. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number FA4887MWBPA is issued as a Request for Quotation (RFQ). This solicitation document incorporated provision and clauses are those effect through Federal Acquisition Circular 2005-50. This solicitation is being issued as a total small business set-aside. The North American Industry Classification System (NAICS) Code is 562219 and the size standard of $12.5 million dollars. The Small Business Competitiveness Demonstration Program does not apply. The Defense Priorities and Allocation System (DPAS) does not apply. The requirement is to procure Medical Waste Disposal Services. The contractor shall provide all labor, tools, supplies, and equipment necessary to provide Management of Hospital Medical Waste Services at 56 MDG/Luke AFB Hospital. FOB Destination to Luke Air Force Base, Arizona 85309. Please see the attached BPA Terms & Conditions, as well as the Performance Work Statement describing what will be required during the duration of the BPA. We request this BPA to begin on 26 May 2011 and last till 26 May 2016. The evaluation will be based on low price, and technically acceptable offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and 52.212-3 Alt I, Offeror Representations and Certifictions-Commercial Items. The clause at FAR 52.212-5 (Dev), Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation.
