- See attached Statement of Work. THIS IS A NOTICE OF INTENT TO SOLE SOURCE. The Agency for Toxic Substances and Disease Registry / Division of Health Studies and the Centers for Disease Control and Prevention (CDC) intends to solicit a single source and award a Cost-Plus-Fixed-Fee contract, in accordance with FAR Part 15 - Contracting by Negotiation and FAR Subpart 6.3 - Other Than Full and Open Competition for a follow up study. The sole source contractor for this requirement is the University of Alabama - Birmingham (UAB), 701 S 20th St., Birmingham, AL 35294. The proposed contract will be to conduct a 36 month follow up study to the Anniston Community Health Survey originally conducted in 2005 - 2007 after preliminary work done by the Anniston Environmental Health Consortium in 2003 - 2005. The study will involve the enrollment of up to 500 participants from the original study and measure dioxin-like compounds in addition to polychlorinated biphenyls (PCBs) to examine associations with heart disease, diabetes, metabolic syndrome and thyroid disease. This contract will pertain to enrollment and physical examination of the participants, collection of data through questionnaire, anthropometric measurements, and collection of biological samples. Collected data and biological samples will be shipped to investigators and laboratories participating in the follow up study. As part of this follow up study, CDC and National Institute of Environmental Health Services (NIEHS) investigators plan to conduct a follow up to the cross-sectional studies conducted by the Anniston consortium in 2005 - 2007. UAB provided the structural framework and staff for the consortium through their Survey Examination Unit and approval of the consortium activities including protocol, informed consents, and questionnaire, and biological samples collection through its Institutional Review Board. UAB completed and maintains the enrollment and data and sample collection for the original Anniston Community Health Survey conducted in 2005 - 2007. CDC anticipates soliciting and negotiating with only one source under the statutory authority 41 U.S.C. 253(c)(1) permitting other than full and open competition in accordance with Federal Acquisition Regulation (FAR) Part 6.302-1 - Only one responsible source and no other supplies or services will satisfy agency requirements. UAB is the only source available to provide services that meet mission critical requirements. The associated NACIS Code is 561990. No Request for Proposal (RFP) will be issued based upon this Notice of Intent. Interested organizations may submit their capabilities/qualifications statement to perform the effort, in writing to Ms. Wikiki Henton via email at kfv5@cdc.gov with the solicitation number (2011-N-13404) referenced in the subject line, no later than 4:00 pm Eastern Time, May 16, 2011.
