- EYE-GLASS/EYE-EXAMINATION SERVICES This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veteran Affairs. The purpose of this Sources Sought notice is to identify technically qualified busineses that can provide on-site and off-site comprehensive eye examinations and Optometric services to veteran beneficiaries. The Department of Veteran Affairs has an ongoing program to furnish eye exams and eyeglasses to those veterans that require them and are eligible for assistance. Any resultant contract is intended to complement Orlando VA Health Care System (OVAHCS) Optometry health services by furnishing on-site and off-site services and therefore, any interested company/provider should be fully licensed and credentialed to provide the required services in accordance with all terms, conditions, provisions, specifications and schedules of an ensuing solicitation. Services shall be performed at the Orlando VA Health Care System at the following locations: 1. Orlando VA Medical Center, 5201 Raymond Street, Orlando, FL 32803 2. Viera VA Outpatient Clinic, 2900 Veterans Way, Viera, FL 32940 3. William V. Chappell, Jr. VA Outpatient Clinic, 5511 National Health Care Drive, Daytona Beach, FL 32114 Responses to this notice should include the following: Name of company/provider; Business size (large or small); Identify whether the company/provider falls into any of the following socioeconomic categories: Service disabled veteran owned businesses; Veteran owned small businesses; 8(a) small businesses; women-owned small businesses, HUBZone small businesses, or Small disadvantaged businesses; and the company/provider's size classification relative to the North American Industry Classification System (NAICS) Code. Please include information on eye glass and/or eye examination services performed at any VA medical centers and/or other government entities, to include contract numbers, dates of performance and points of contact. Include any other pertinent information which demonstrates the company/provider's ability to meet the above requirement. This is NOT a solicitation for proposals, proposal abstracts, or quotations; no formal solicitation document exists at this time. Responses to the information requested will assist the Government in determining the appropriate acquisition method, including if a set-aside is possible. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis/solicitation may be published in the Federal Business Opportunities. Therefore, responses to this notice will not be considered adequate responses to a solicitation. CONFIDENTIALITY: No proprietary, classified, confidential, or sensitive informatin should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
