- Based on responses to RFQ VA-69D-11-RQ-0757 entitled "Warming System" for the Edward Hines VA Hospital, Hines, IL," award has been made to Medical Solutions, Incorporated, Inc. at the total contract amount of $75,175.00 as lowest price technically acceptable to the Government. Sincerely, Scott Ivy Contracting Officer Great Lakes Acquisition Center (GLAC) Milwaukee, WI JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION 3 Liter Bag Warming System, Hines VA 1.Identification of the Agency and the Contracting Activity: Department of Veterans Affairs, Great Lakes Acquisition Center, 5000 W National Avenue Bldg 5, Milwaukee WI 53295 2.Nature and/or description of the action being approved: Justification and approval for other than full and open competition for the purchase of a 3 Liter Bag Warming System for the VA Hospital in Hines, IL. This procurement will proceed on a sole source basis with Medical Solutions, INC. 3.A description of supplies/services required (including the estimated value): The VA Hospital in Hines, IL, has a requirement for a one time purchase of a 3 Liter Bag Warming System used for warming various fluid like I.V fluids or various fluids of the body. The approximate cost is $79,950.00 4.Statutory authority permitting other than full and open competition: FAR 6.302-1(a)(2). The VA's minimum needs can only be satisfied by unique supplies or services available from only one source. (41 U.S.C. 253 (c)(1)). 5.Facilities Unique Qualifications: The VA Hospital in Hines, IL requires this exact model because it is the only model on the market that fulfills the hospitals needs of the following; The only product that has monitors and displays the actual fluid temperature of each bag, monitors and displays the actual time bag has been warming, rapidly warms three liter bags in about an hour, the documenter assists in compliance with regulatory standards and fluid manufacturers' recommendations, and provides Time/Date Stamp Technology, it has the capacity of holding (8) 3000 ml bags, and lastly the locking cabinet assists in compliance with regulatory standards. This vendor is the only one that makes a irrigation warmer for a 3 liter bag of fluid. 6.Efforts made to ensure that offers are solicited from as many potential sources as is practicable: A search of GSA, SBA and Vetbizops did not reveal another system with the characteristics that Ms. Miller has requested. GSA had 2 similar but did not have what Hines needed. I sent 3 FSS vendor contracts to Hines staff for their review. Hines staff determined that the 3 vendors FSS contracts did not contain an item that could meet their needs. A FedBizOpps publication will announce the intent to sole source this requirement. 7.Determination by the Contracting Officer that the anticipated cost to the Government will be fair and reasonable: Overall pricing will be compared to contractor's list prices. 8.Market survey conducted: Barbara Miller, has indicated that this is a sole source requirement/need that is critical to their department and vital to direct patient care, throughout the Department of Veterans Affairs Agency. The Contracting Officer researched GSA, FBO, SBA and Vetbiz to verify this is a Sole Source. 9.Other facts: Exact model was previously purchased as a sole source and there were no objections from contractors/vendors to the FBO posting and SSJ. 10.Other sources expressing an interest: A FEDBIZOPPS ad may generate alternate sources. 11.Actions to remove or overcome any barriers to competition: The Contracting Officer searched the Dynamic Small Business Search feature of CCR and VetBiz Registry and located 3 small businesses but a search of their schedules did not reveal comparable products. 12.Certification: I certify that this justification is accurate and complete to the best of my knowledge.
