Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: VA-528-11-RQ-0157 This requirement is to provide a Rotary Chair Med 4, for complete kinetic testing. Line ItemDescriptionTotal Cost 1(1)Rotary Chair Med 4 to include: "Remote control for manual operation "Automatic operation by the Ulmer's VNG Software "Ability to pre-define up to 16 profiles (sinusoidal, SHA test, square, etc) "Stimulation profiles can be modified upon demand "VOR gain, phase, preponderance and multi-frequency analysis "High-speed rotational velocity up to 200 °/s "Acceleration of 100 °/s ² "Gas control for seat back adjustment "Weight/load of 130Kg (260 pounds) "Shoulder/seat harness for patient safety$ 2Shipping and Handling$ TOTAL$ *Pricing must include: Installation, all parts and labor for installation, (2) user/service manuals, and a minimum 1 year warranty on equipment and parts. Place of Performance: Department of Veterans Affairs Bath VA Medical Center (VISN 2) 76 Veterans Avenue Bath, NY 14810 Delivery required NLT 60 days from date of award. The Rotary Chair Med 4 is provided on a commercial basis. This procurement is restricted to Service-Disabled, Veteran-Owned Small Businesses (SDVOSB). Per VAAR deviation, 804.1102, all SDVOSBs must be verified by the Office of Small and Disadvantaged Business Utilizations Center for Veterans Enterprise (CVE) through www.vetbiz.gov to before award. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-2, Evaluation of Commercial Items, applies to this solicitation. Evaluation will be based on Technical, Price, and Delivery. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its proposal or has completed them on-line (https://orca.bpn.gov/). A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The offeror shall also provide its Commercial and Government entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of the information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
