REQUEST FOR PROPOSAL Ventilation The South Bronx Job Corps Academy is a 501c3 federally funded Vocational and Educational Training Program that services youth in a residential setting. The program operates year round with approximately 200 residential and 100 non-residential trainees at the South Bronx Job Corps Academy; located at 1771 Andrews Avenue, Bronx New York 10453. This program is run by ResCare, Inc. for the United States Department of Labor. SCOPE OF WORK Deficiency 73607 Findings : Combustion air not provided Responsible Funded findings: The building has a mechanical room with a non-direct-vented gas-fired appliance. The mechanical room has no provision for combustion air. (IMC, 2006, 703.1 or IFGC, 2006, 304.6.2) Recommendations: Provide mechanical room with two openings for outdoor combustion air, one within 1 foot of the ceiling and one within 1 foot of the floor. The size of the openings shall be 12" x 12". Provide louvers and insect screens to protect openings. Deficiency-73615 Findings: The storage room in the attic is approximately 450 SF. It gets extremely hot and humid in summer. The maintenance staff frequently accesses the room. Excessive temperature and humidity is a major cause of reducing the life of items stored. Recommendations: Provide a wall-mounted 300 CFM exhaust fan complete with a back draft damper and a remote wall-mounted thermostatic switch. Provide 12"x12" inlet damper with insect screen to allow for makeup air. Energize fan from nearest suitable source using 120V, 1-phase wiring including 40 LF of 3 #12 in 1/2" EMT. Install a local single-pole switch near the unit for servicing. Deficiency-61334 Exhaust ventilation not provided in toilet room/janitor closet Findings: Toilet room located at the third floor conference room does not have a means for exhaust ventilation. Recommendations: Provide a 100 CFM wall exhaust fan with integral back draft damper and 10 LF of 6" diameter duct. Extend the duct 1 LF through exterior wall and provide appropriate wall cap. Energize from lighting circuit, using the existing switch and 15 LF of 3 #12 in 1/2" EMT. Provide for drywall repair if necessary. Notes: As of the February 2009 Facility Survey, this deficiency was still valid. Deficiency 61334 Deficiency- 73604 Air curtain not provided at Exterior kitchen exit Findings : The exterior door at the kitchen exterior is not self-closing and has no air curtain. This allows insects to enter the building whenever the door is opened. Recommendations: Install a 48-inch wide air curtain above the door. Provide power from the nearest suitable source using 50 LF of 3 #12 wire in 1/2 EMT. Interlock the operation of the fan with the opening of the door. Provide a 1-pole switch near the unit for servicing. Each Deficiency will be award individually. Must provide a copy of current licensure and insurance LENGTH OF AGREEMENT Contractor must provide scope of work and time frame in which work is to be completed. Scope of work must be written to that listed above. All bidders must be in Cost Break down format. NO replacements of Substitutions allowed. ACKNOWLEDGEMENT OF ACCEPTANCE •· Contractor must acknowledge by signature that they meet all Federal, State, Local and OSHA regulations and that all specifications terms and conditions set forth are acceptable. Bidders must submit their proposal in a sealed envelope by 5 p.m. close of business June 24,2011 Purchasing Agent. The following documents Davis Bacon Act (40 U 27a Attachment B), Certification of Non-Segregated Facilities Pursuant to FAR 52.222-21 (Attachment C), Equal Opportunity and Affirmative Action Requirement Pursuant to FAR 52.222-26 and FAR 52.222-27, FAR 52.252-2, 52.204-1, 52.215-19, PRC 4121 and 4170 will be incorporated and made part of agreement.
