Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-248-11-RQ-0629 0001 and it is being issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those effective through Federal Acquisition Circular 2005-50, effective 16 Mar 2011. Interested persons may identify their interest and capability to respond to the requirement or submit proposals, all proposals received prior to 06/10/2011 will be considered by the Government NAICS code is 423450, size standard 100 employees. This configuration expands treatment options for the anterior segment. Include Selective Laser Trabeculoplasty for open-angle glaucoma, Two procedures, one laser system, at the touch of a button, switch between Nd: Yag and SLT. Replacement of material and parts will be furnished to the Government at the point of installation, if installation is within the continental United States, or f.o.b. the continental U.S. port to be designated by the contracting officer if installation is outside of the continental United States.
